What the evidence adds up to

The four abstracts cover vulvar neoplasia broadly, from benign precursors to invasive cancer, but none reports a drug repurposing trial or any pharmacological intervention. Vulvar intraepithelial neoplasia (VIN) is described as a premalignant skin condition. A two-year prospective study of eight women with newly diagnosed VIN found that careful observation without primary surgery was not feasible: six of the eight required immediate treatment, and by study end seven had undergone surgical treatment, mostly for symptom control. The same study reported that women scored poorly on quality of life and sexual functioning measures, and this did not appear to be driven by depression or anxiety.

For invasive vulvar cancer, the most common subtype is squamous cell carcinoma. Treatment is predominantly surgical, though concurrent chemoradiation is described as an effective alternative for advanced tumours. One review states that treatment ranges from wide local excision with or without lymph node biopsy to radiation therapy with chemo- or immunotherapy, but no specific drug, dose, or repurposing candidate is named. A separate research project developed a patient-reported outcome measure for women after surgical treatment of vulvar neoplasia. It identified 31 symptoms and the degree of distress they caused during the first seven days after discharge, and concluded that including patient self-report in follow-up care could enhance supportive care quality. No drug treatment was tested or proposed.

What is missing is any clinical trial testing a repurposed drug for vulvar benign neoplasm or VIN. The evidence base consists entirely of surgical and observational studies. No abstract provides response rates, survival curves, or drug names. A repurposing trial would require funding, a defined drug candidate, and a design that accounts for the rarity of the condition and the need for patient stratification by histology and symptom burden.

Cancer of the vulva

AbstractVulvar cancer is an uncommon gynecological malignancy primarily affecting postmenopausal women. There is no specific screening and the most effective strategy to reduce vulvar cancer incidence is the opportune treatment of predisposing and preneoplastic lesions associated with its development. While vulvar cancer may be asymptomatic, most women present with vulvar pruritus or pain, or have noticed a lump or ulcer. Therefore, any suspicious vulvar lesion should be biopsied to exclude invasion. Once established, the most common subtype is squamous cell carcinoma. Treatment of vulvar cancer depends primarily on histology and surgical staging. Treatment is predominantly surgical, particularly for squamous cell carcinoma, although concurrent chemoradiation is an effective alternative, particularly for advanced tumors. Management should be individualized, and carried out by a multidisciplinary team in a cancer center experienced in the treatment of these tumors.

Cancer of the Vulva: A Review

AbstractBACKGROUND: Vulvar cancers, although rare, are becoming an increasingly serious threat to women's health. Cancer of the vulva accounted for 0.3% of all new cancers in the United States in 2019, with 6,070 newly diagnosed cases. This review details the epidemiology, pathogenesis, diagnosis, staging, and treatment of vulvar malignancies. OBJECTIVE: To review cancer entities of the vulva, including vulvar intraepithelial neoplasms, squamous cell carcinoma (SCC), malignant melanoma, basal cell carcinoma, neuroendocrine tumors, and adenocarcinomas. MATERIALS AND METHODS: Literature review using PubMed search for articles related to cancer of the vulva. RESULTS: Vulvar intraepithelial neoplasms represent premalignant precursors to SCC of the vulva. There are several different histopathologic subtypes of SCC, and treatment is dependent on characteristics of primary tumor and lymph node involvement. Melanoma is the second most common cancer to affect the vulva, and staging is based on tumor, node, and metastatic spread. CONCLUSION: Vulvar malignancies are rare, and diagnosis is dependent on biopsy and pathologic evaluation. Treatment for vulvar malignancies depends on histopathologic diagnosis but ranges from wide local excision with or without lymph node biopsy or dissection to radiation therapy with chemo- or immunotherapy. Overall survival varies by diagnosis.

The prospective management of women with newly diagnosed vulval intraepithelial neoplasia: Clinical outcome and quality of life

AbstractVulval intraepithelial neoplasia (VIN) is a premalignant skin condition with significant morbidities from standard surgical treatment. This two-year prospective study recruited women with newly diagnosed VIN for careful observation rather than primary surgical excision. Surgery was performed to exclude progression or for uncontrollable symptoms. Participants were seen six-monthly for clinical review and completion of questionnaires assessing depression and anxiety, quality of life, sexual functioning, and partner relationships. Eight women were recruited. Six women required immediate treatment and two women were observed carefully. By study completion, seven participants had undergone surgical treatment, often for symptom control. Women scored poorly on quality of life and sexual functioning scores; this did not appear to be due to depression or anxiety. In conclusion, careful observation does not appear a feasible option for most women with VIN since the majority require surgical treatment. VIN has an adverse impact on quality of life and sexual functioning.

Vulvar Fibromatosis

AbstractOBJECTIVE: To present a case of recurrent vulvar fibromatosis in an adolescent, discuss the specific difficulties of treating adolescents, and review the literature on available treatment. METHODS: We present a case of recurrent vulvar fibromatosis in a 14-year-old girl, requiring several treatment modalities, including multiple surgeries, radiation therapy, and multiagent chemotherapy. We then discuss management strategies for these tumor types, and specifically examine how tumor location may impact their treatment. RESULTS: Vulvar desmoids are extremely uncommon and they can be disfiguring and cause significant discomfort for women. Initial management of these tumors is surgical excision, yet failed surgery is often followed by other treatment modalities, including radiation, tyrosine kinase inhibitors, nonsteroidal anti-inflammatory drugs, hormonal therapy, and chemotherapy. This case clearly highlights the difficulties in managing these rare tumors, particularly in the adolescent population. CONCLUSIONS: Desmoid tumors are nonmalignant, locally aggressive neoplasms most common in the 15 to 60 years age group. They are associated with high estrogen states, prior surgical trauma, and Gardner syndrome. Most commonly, desmoid tumors present in the abdominal wall, shoulder, neck, and chest, but can occur anywhere in the body. Given their rarity and lack of definitive therapy, vulvar desmoid tumors can be exceedingly difficult to treat, and are best managed with an interdisciplinary approach.

Patient-reported Outcome Measures for Women with Vulvar Neoplasia – Current Situation and Future Directions

AbstractThis article discusses concerns about women with vulvar neoplasia (vulvar intraepithelial neoplasia and vulvar cancer) and the available surgical treatment options. Given the gaps in the evidence base in terms of women with vulvar neoplasia and surgical treatment the women with vulvar Neoplasia-Patient Reported Outcome (WOMAN-PRO) research project focused on complications, symptoms and associated distress of women with surgically treated vulvar neoplasia. The main results of the research project are summarised, showing complications identified by clinicians, experiences reported by patients, the newly developed WOMAN-PRO instrument and symptom occurrence of each of 31 symptoms and the degree to which symptoms distressed women during the first seven days after discharge following surgical treatment for their vulvar neoplasia. Furthermore, based on the major findings of the project, suggestions for further research and clinical practice and conclusions are presented. We conclude that including patient self-report as a major element in follow-up care has the potential to enhance the quality of supportive care.
