What the evidence adds up to

A 2013 cross-sectional survey of 89 patients with trigeminal nerve injury (56 inferior alveolar nerve injury, 33 lingual nerve injury) found that 86% reported pain associated with their nerve injury. Patients with severe pain showed elevated depression and pain catastrophising, substantially reduced quality of life and coping efficacy. Pain intensity uniquely contributed between 17% and 26% of the variance in predicting pain catastrophising, depression, coping efficacy, and generic and oral health-related quality of life, but was not a significant predictor of anxiety. The authors concluded that traumatic injury to the trigeminal nerve carries substantial patient burden, particularly when severe neuropathic pain is present.

A 2010 case report described a 62-year-old woman with numbness and pain over the left lower lip and chin after inferior alveolar nerve injury. She received a single subcutaneous injection of botulinum toxin type A into the chin area. At one month the affected area had decreased in size; at two months the patient reported a slight decrease in pain. The authors called for a randomised controlled study with a large number of patients. No further controlled data from that study have been reported.

Two 2024/2025 overview articles describe trigeminal neuralgia as a rare condition characterised by sudden, unbearable, paroxysmal, stabbing pain, usually unilateral, along trigeminal dermatomes. They state the disorder is not lethal but is debilitating and can lead to psychological disorders. Both articles review pharmaceutical management, surgical management, and alternative treatments, and discuss newer guidelines. A 2023 article on treatment in the elderly notes that clinicians are dissatisfied with current results, and that research focuses on improving drug therapy with new drugs and combinations, or developing surgical techniques that affect the trigeminal nerve system, each method targeting a single link in the pathogenetic chain.

What is still missing are large, randomised controlled trials for botulinum toxin in trigeminal nerve injury pain, any drug or intervention shown to improve quality of life or reduce pain in a controlled setting beyond single case reports, and a treatment approach that addresses the multiple pathogenetic links simultaneously rather than one at a time. Patient stratification by pain severity, injury type, and psychological comorbidity has not been used to guide treatment selection in any published trial.

The Psychosocial and Affective Burden of Posttraumatic Stress Neuropathy Following Injuries to the Trigeminal Nerve

AbstractAIMS: To explore the impact of trigeminal nerve injuries on quality of life, including the effect of pain on psychological and affective function. METHODS: An observational, cross-sectional survey design was employed. Fifty-six patients with inferior alveolar nerve injury (IANI) and 33 patients with lingual nerve injury (LNI) completed standardized self-report measures of pain intensity, pain catastrophizing, self-efficacy to cope with pain, and mood, in addition to generic and oral health-related quality of life (HRQoL) indicators. The impact of pain severity on these aspects of psychosocial function was examined. Summary statistics were calculated for all measures and compared with norms or values of other relevant studies, when available, using t tests. The impact of pain severity on these aspects of psychosocial function was examined using analysis of variance and hierarchical multivariate regression models. RESULTS: The majority of patients reported pain associated with their nerve injury (86%). Nerve injury had a significant impact on all investigated domains, and this was closely linked with reported pain levels. Patients with severe pain showed particularly elevated levels of depression and pain catastrophizing, as well as substantially reduced HRQoL and coping efficacy levels. Pain intensity level was a significant predictor in all models except anxiety, uniquely contributing between 17% and 26% of variance to the prediction of pain catastrophizing, depression, coping efficacy, and generic and oral HRQoL. CONCLUSION: Traumatic injury to the trigeminal nerve is associated with a substantial patient burden, particularly in patients who experience severe neuropathic pain as part of their condition. These findings highlight the need to identify, develop, and evaluate more effective treatments for neuropathic pain in trigeminal nerve injury that will not only provide clinically meaningful reductions in pain but also improve patients' quality of life.

Use of Botulinum Toxin Type A Injection for Neuropathic Pain after Trigeminal Nerve Injury

AbstractOBJECTIVE: To present a case that neuropathic pain following traumatic injury of the inferior alveolar nerve, which was relieved by the injection of BTX-A. DESIGN: Case report. SETTING: Tertiary care University hospital. SUBJECT: A 62-year-old female was referred by her general dentist to our clinic due to numbness and pain over the left side of her lower lip and chin region. INTERVENTION: Botulinum toxin type A injected into the middle of chin area subcutaneously. RESULTS: At 1 month after BTX-A injection, the affected area had decreased in size. And at 2 months, the patient reported a slight decreased in pain, and CPT differences being sustained at a reduced level. CONCLUSIONS: This case report suggests an effective new modality for treating neuropathic pain after trigeminal nerve injury. A further randomized controlled study involving a large number of patients is needed.

Approaches to the Systematization of the Trigeminal Nerve Disorders

AbstractPurpose: To systematize secondary neurogenic lesions of the trigeminal nerve branches. Patients and Methods: We observed 1096 patients with secondary neuronal lesions of the trigeminal nerve system. Results: The systematization of lesions of trigeminal nerve branches is proposed, taking into account the mechanisms of development, the course of the disease, secondary complications and localization. Conclusion: Systematization can be recommended for doctors to improve the diagnosis and treatment of diseases of the trigeminal nerve system.

Method of treatment of trigeminal neuralgia in the elderly

AbstractThe treatment of trigeminal neuralgia (HTN) in the elderly, highlighted in the literature, reflects the dissatisfaction of clinicians with the results of the treatment of this chronic disease. Scientific research on the treatment of trigeminal neuralgia in the elderly is carried out by improving drug therapy through the introduction of new drugs and their combinations and through the development of new surgical techniques that affect the trigeminal nerve system (TN). Each of the therapeutic methods is mainly aimed at a single link in the pathogenetic chain of the development of trigeminal neuralgia, either to increase the threshold of excitability of sensitized neurons of the brain stem and cortex or to turn off the trigger zone.

To ease the pain and suffering patient’s agony, a precise diagnosis and an effective therapeutic approach are essential. A comprehensive examination of the diagnosis, management, and rehabilitation of trigeminal neuropathy is given in this article. This includes pharmaceutical administration, surgical management, alternative recently discovered methods of treatment and a screening approach. Latest innovations and newer guidelines of treatment are also discussed to enhance clinical outcomes and improve patients’ prognoses.
