What the evidence adds up to

A 2003 review notes that tonsillectomy remains common in the United States despite stricter surgical criteria, and that primary care physicians are central to diagnosing and managing tonsillitis. A 2019 retrospective Israeli study reported a clear reduction in tonsillitis episodes and healthcare costs after tonsillectomy in adults, though a 2014 Cochrane review had not definitively answered the question of effectiveness for adults, only suggesting a positive impact on quality of life.

A 2021 randomised, open-label, multicentre study of 238 outpatients aged 6 to 18 years with acute non-bacterial tonsillitis compared the phytoneering extract BNO 1030 (Imupret) plus standard symptomatic treatment against standard treatment alone. The study reported a significant reduction in the severity of local symptoms and general condition at each control point over one year, and a 66.56% decrease in the recurrence rate of tonsillitis. No adverse reactions were noted. The trial was registered retrospectively in the German Clinical Trials Register.

A 2018 study investigated the complex homeopathic medication Tonsilotren in patients aged 6 to 60 years with moderate recurrent tonsillitis, set against the background of watchful waiting recommendations. A 2023 prospective clinical study of 30 children aged 4 to 12 years with at least five episodes of tonsillitis in the previous year tested homoeopathic medicines selected according to the totality of symptoms. Twenty-nine of the 30 cases showed a reduction in the number of episodes over one year, with a p value indicating statistical significance. The study noted exacerbating factors including exposure to cold, citrus fruits, chocolates, milk products, and preservative-containing foods.

What is still missing are large, independently funded, blinded randomised trials comparing these non-surgical options directly against watchful waiting or placebo, with clearly defined patient stratification by age, bacterial versus non-bacterial aetiology, and baseline episode frequency. The 2021 study was open-label and the 2023 study had no control group. No trial has yet established whether any of these interventions reduce the need for tonsillectomy in the long term.

The primary care physician is integrally involved in the diagnosis and treatment of tonsillitis, the chief reason for tonsillectomy. This article gives guidelines for diagnosis and management of tonsillitis and provides an overview of its potential complications.

A randomized, open-label, multicentre, comparative study of therapeutic efficacy, preventive potential and tolerability of BNO 1030 extract, containing Althea root, Chamomile flowers, horsetail herb, walnut leaves, yarrow herb, oak bark, dandelion herb in the treatment of acute non-bacterial tonsillitis in children aged 6 to 18 years

AbstractAbstract Acute tonsillitis tends to recur. In cases where patients do not meet the Paradise criteria, the possibilities of non-surgical treatment are more often considered. The objective of this study was to evaluate the therapeutic efficacy during the long-term follow-up and the effect on the recurrence of the phytoneering extract BNO 1030 (Imupret®) in patients with acute non-bacterial tonsillitis. Methods In this Randomized, Open-Label, Multicentre, Comparative Study, 238 outpatients aged 6–18 years were randomized to receive either BNO 1030 (Imupret®) for 4 weeks in addition to standard symptomatic treatment, or to receive standard treatment. Evaluation criteria: reduction in the symptom severity less than 1 point, the number of tonsillitis recurrences at each control point after 3, 6 and 12 months during the one-year follow-up. Results A significant reduction in the severity of local symptoms and the general condition at each control point within the year of follow-up and a significant decrease (by 66.56%) in the recurrence rate of tonsillitis were noted. The anti-recurrent action was manifested during within the year of follow-up. All patients tolerated phytotherapy well; no adverse reactions were noted. Conclusions BNO 1030 (Imupret®) is a safe and effective medicinal product for acute non-bacterial tonsillitis in children aged 6–18 years. In addition to the main symptomatic treatment, it leads to a significant reduction in the clinical manifestations and the number of recurrences of tonsillitis within the year of follow-up. A clinical study on homoeopathic treatment of recurrent tonsillitis based on stuart close’s concept of susceptibility and it’s role in selection of potency

AbstractTonsillitis is a common childhood infection and recurrent episodes have long term side effects and cause significant morbidity. It is a common cause of absenteeism from school. Like any other operation, tonsillectomy is not without risks and the rising resistance to antibiotics has led to a demand for alternative options. Recurrence of a disease shows a morbid susceptibility and homoeopathic medicines chosen according to the totality of symptoms in suitable potency can remove it. The scientific community now has called for extensive research in the study of genes involved in transferring susceptibility to certain diseases. The aim of my study is to study the effectiveness of homoeopathic medicines in treating recurrent tonsillitis and thereby evaluate Stuart Close’s concept that similimum can satisfy morbid susceptibility to a disease and hence reduce its recurrence. A prospective clinical study was conducted in Govt. Homoeopathic Medical College, Thiruvananthapuram. 30 cases satisfying the diagnostic criteria in the age group of 4-12 years were taken. Those who had minimum of 5 or more episodes in the last year were chosen and after detailed case taking homoeopathic medicine was administered. The period of study was 1 year. Study design is experimental before and after-one group. Outcome of the treatment was assessed by noting the reduction in the number of episodes of tonsillitis. Final evaluation of the data done by Wilcoxon signed rank test. 29 out of 30 cases showed reduction in the number of episodes of recurrent tonsillitis when homoeopathic medicines were administered.
