What the evidence adds up to
Spindle epithelial tumor with thymus-like differentiation (SETTLE) of the thyroid is a rare neoplasm, with roughly 50 cases reported in the international literature as of 2022. It occurs predominantly in young patients and is considered a low-grade malignant tumor with indolent growth, but a tendency to develop delayed blood-borne metastases years after diagnosis. One 1999 case report describes a 29-year-old man whose tumor showed numerous mitotic figures and focal necrosis, features the authors note had not been previously reported and which they suggest may represent an aggressive variant. A 2010 report of two cases provided follow-up of 64 months and 30 months, respectively, without specifying recurrence or metastasis.
Diagnosis is frequently delayed. A 2021 case of a 7-year-old girl reports that fine needle aspiration cytology raised a differential of medullary thyroid carcinoma and small round blue cell tumour, and a definitive diagnosis of SETTLE was reached only after additional investigations and total thyroidectomy. The authors state that this diagnostic difficulty influenced choice of treatment. A 2022 case describes a 41-year-old man whose tumor occurred in the soft tissues of the neck around the thyroid, rather than within the gland itself. Immunohistochemistry showed CK and CK19 positivity in thymic epithelial components and CK, vimentin, and Bcl-2 positivity in spindle cells. This patient underwent mass resection, and no recurrence or metastasis was observed at 8 months postoperatively.
Surgical resection is reported as the main treatment option. No abstract mentions any drug therapy, chemotherapy, radiotherapy, or targeted agent being tested or used for SETTLE. No response rates, survival data from controlled studies, or comparisons between treatments are available in these reports. The evidence consists entirely of case reports and small case series.
What is missing is any clinical trial, any systematic collection of treatment outcomes across centres, any prospective registry, and any funding for such work. Because the tumour is so rare, no randomised or even large observational study exists to guide management beyond surgery. Patient stratification by mitotic activity, necrosis, or molecular markers has not been attempted in a formal study.

AbstractSpindle epithelial tumor with thymus-like differentiation (SETTLE) is a rare, apparently low-grade spindle cell tumor of the thyroid gland occurring in young individuals and thought to be derived from thymic or branchial pouch remnants. Spindle epithelial tumor with thymus-like differentiation has little to no mitotic activity, and focal necrosis has been reported in one case. We present a case of SETTLE in a 29-year-old man that was initially evaluated by fine-needle aspirate biopsy and ultimately found to be consistent histologically with SETTLE. In this case, there were numerous mitotic figures among the spindle cells and focal necrosis. Spindle epithelial tumor with thymus-like differentiation has been considered to be a tumor of low malignant potential with metastases developing some years after diagnosis. This is the first case in which prominent mitotic activity and necrosis is reported perhaps representing an aggressive variant.

AbstractA 7-year-old girl presented with a painless neck swelling localised near the left lobe of the thyroid gland, which was initially investigated by fine needle aspiration cytology. This raised a differential diagnosis of medullary thyroid carcinoma and small round blue cell tumour. Only after several additional clinical investigations and a total thyroidectomy was a definitive diagnosis of spindle cell tumour with thymus-like differentiation (SETTLE) reached. This case report highlights how contemporaneous clinical and investigation findings made arriving at a definitive diagnosis challenging, contributed to diagnostic delay, and ultimately influenced choice of treatment.

Spindle epithelial tumor with thymus-like differentiation: A case report and literature review*
AbstractAbstract Spindle epithelial tumor with thymus-like differentiation (SETTLE) is a rare, low-grade malignant neoplasm that mainly occurs in the thyroid gland or its perimeter. Fewer than 10 cases of SETTLE have been reported in the Chinese literature, whereas approximately 50 cases have been reported in the international literature. We report a rare case of SETTLE that occurred in the soft tissues of the neck around the thyroid gland in a 41-year-old man. The neoplastic tissue, which mainly consisted of spindle epithelial components, included sporadically visible thymus-like structures. Immunohistochemistry demonstrated CK and CK19 positivity in the thymic epithelial components and CK, vimentin, and Bcl-2 positivity in the spindle cells. Surgical resection is the main treatment option for SETTLE. This patient underwent mass resection of the neck, and no recurrence or metastasis was observed even at 8 months postoperatively.
