What the evidence adds up to
Synovial plica syndrome occurs when a normal embryonic remnant in the knee becomes painful after injury or overuse, presenting as anterior knee pain that can be difficult to diagnose. The suprapatellar plica is the most common type but is usually asymptomatic; in 3 to 30% of cases plicae become symptomatic. One case report describes a patient with spontaneous knee pain and swelling caused by an atraumatic hematoma trapped in the suprapatellar bursa by a complete suprapatellar plica, a presentation the authors state has not been described before.
A 2024 case report describes an avid runner whose synovial plica syndrome pain was refractory to arthroscopic plica excision, physical therapy, nonsteroidal anti-inflammatory drugs, and intraarticular steroid injections. He received three rounds of lumbar sympathetic block, which resulted in significant and sustained pain relief. The authors suggest this indicates a sympathetic component to the condition. This is a single case, not a controlled trial.
No controlled trials of lumbar sympathetic block for synovial plica syndrome exist. The evidence for any treatment remains limited to case reports and reviews. What is missing is prospective, controlled studies with adequate sample sizes, standardised diagnostic criteria, and clear outcome measures that distinguish which patients might benefit from sympathetic blockade versus other interventions.

AbstractSynovial plica syndrome (SPS) occurs in the knee, when an otherwise normal structure becomes a source of pain due to injury or overuse. Patients may present to general practitioners, physiotherapists, or surgeons with anterior knee pain with or without mechanical symptoms, and the diagnosis can sometimes be difficult. Several studies have examined the epidemiology, diagnosis, and treatment of SPS. We review these resources to provide an evidence-based guide to the diagnosis and treatment of SPS of the knee.
Lumbar Sympathetic Block for Treatment of Knee Pain From Synovial Plica Syndrome: A Case Report
AbstractLumbar sympathetic blocks (LSBs) are used to treat sympathetically mediated pain in the lower extremities, kidneys, ureters, and genitals. LSBs use local anesthetic to block the sympathetic system to modulate pain response. In this case report, an avid runner was diagnosed with synovial plica syndrome. His pain was refractory to arthroscopic plica excision, physical therapy, nonsteroidal anti-inflammatory drugs, and intraarticular steroid injections. He received 3 rounds of LSB resulting in significant and sustained pain relief. This case suggests that LSB successfully treated knee pain from synovial plica syndrome and there may be a sympathetic component to this disease state.

Undercover hematoma: A rare presentation of a complete suprapatellar plica
AbstractSynovial plicae are remnants of synovial septa that fail to fully resorb during embryonic knee development. In 3 to 30% of cases, they can become symptomatic (synovial plica syndrome), typically presenting with anterior knee pain exacerbated by knee flexion. Plicae are classified based on their location, with the suprapatellar being the most common but typically asymptomatic.This article describes a clinical case of spontaneous knee pain and swelling revealed to be secondary to an atraumatic hematoma confined to the suprapatellar bursa by the presence of a complete suprapatellar synovial plica. According to the authors' research, there are no described cases with a similar presentation.
