What the evidence adds up to

A 1982 study of 742 patients treated in six programmes and evaluated at six-month follow-up found significant and pervasive improvements in alcohol and drug use, employment, criminal behaviour, and psychological function. Patients undergoing long-term treatment showed greater improvement and better six-month outcomes than those undergoing short-term therapy on 12 of 18 criteria. The authors concluded the data provided evidence for the therapeutic benefit of substance abuse treatments, but noted that few studies had adequately assessed effectiveness using an appropriate range of reliable outcome measures.

A 2006 review of large-scale national evaluation studies, including the Drug Abuse Treatment Outcomes Studies, stated that such studies provide a broad view of treatment services in community-based programmes but do not address the implementation or effectiveness of specific clinical treatment approaches. What is still missing are adequately powered randomised controlled trials that compare specific medications or treatment durations against placebo, reliable biomarkers to stratify patients by genetic or metabolic profile, and funding for long-term follow-up studies that can distinguish treatment effects from natural recovery.

Is Treatment for Substance Abuse Effective?

AbstractFew studies have assessed adequately the effectiveness of alcohol and drug abuse treatments using an appropriate range of reliable outcome measures, a representative sample of alcohol and drug abuse treatment modalities, or more than one perspective on treatment effectiveness. This article evaluates substance abuse treatment using a sample of 742 patients treated in six programs and evaluated at six-month follow-up. The following three major questions were addressed: (1) do patients improve following treatment; (2) are improvements confined to alcohol or drug use, or are they more pervasive; and (3) are these improvements a result of treatment? The results indicated significant and pervasive improvements in virtually all areas for both alcoholics and drug addicts. Major changes were seen in alcohol and drug use, employment, criminal behavior, and psychological function. Patients undergoing long-term treatment showed greater improvement and better six-month outcomes than those undergoing short-term therapy on 12 of 18 criteria. The data provide evidence for the therapeutic benefit of substance abuse treatments.

Substance abuse among nurses—Intercession and intervention

AbstractIT IS NOT EASY to report a coworker who may have a substance abuse problem, so many nurses choose to remain silent about this issue. THIS ARTICLE PROVIDES suggestions for staff nurses about how to confront a peer, document inappropriate nursing behaviors related to substance abuse, and report these issues to a manager. The manager's role in counseling and intercession with a substance abusing employee also is detailed. REMEDIATION AND SUPPORT programs are addressed along with return-to-work issues and the need for education about this debilitating disease.

The Drug Abuse Treatment Outcomes Studies: outcomes with adolescent substance abusers

AbstractLarge-scale studies of drug abuse treatment effectiveness provide benchmarks that can be used to gauge drug abuse treatment services at a given point in time, as well as over time. Such large-scale studies have inherent strengths and weaknesses: they provide a broad view of the nature of treatment services offered in community-based programs, yet, because of their breadth, they do not address the implementation or effectiveness of specific clinical treatment approaches. Consequently, national evaluation studies are best suited to examining global questions about the characteristics of individuals who utilize substance abuse treatment across a broad sample of settings and the outcomes associated with their treatment participation. Findings from these studies can help to advance the field by identifying gaps in existing treatment systems and, by informing the development of both clinical and policy approaches, to improve the quality of treatment provided.

AbstractMedication has become an increasingly accepted adjunct to other forms of drug abuse treatment. Its acceptance is due in part to clinicians' frustration in treating crack cocaine abusers and to increasing integration of drug abuse treatment into mainstream medicine. A rapidly growing body of literature attests to clinical researchers' interest in pharmacotherapy of substance abuse. Medications can be a useful adjunct to drug abuse treatment in many different ways. The clinical challenge is to decide who should be treated with what medication and at what stage of recovery medications are helpful. This article discusses medications currently in use for treatment of addictive disease, describes some aspects of medication development, and suggests more cost-effective methods of exploring the efficacy of medications for treatment of drug abuse.

AbstractReviews the book, Behavioral and Biochemical Issues in Substance Abuse edited by Frank R. George, Doris Clouet, and Barry Stimmel (1991). The determinant role of genetics on behavioral and biochemical responses in substance abuse is an extremely interesting and increasingly pertinent area of research. This is the main topic and focus of this book. This book provides an excellent overview of the power of genetic experimental designs, the results that can be generated as well as the cautions that must be observed in this approach. The editors have combined an independent collection of works in a cohesive, enlightening, thorough text that is an asset to the substance abuse field.
