What the evidence adds up to

Basosquamous carcinoma is a rare skin tumour, comprising 1.7 to 2.7 percent of all skin carcinomas in one series of 4,235 basal cell carcinomas, 1,519 squamous cell carcinomas, and 288 basosquamous carcinomas. In that series, 70.5 percent of basosquamous carcinomas were located on the head and neck, particularly the nose, forehead, and cheeks, and they represented nearly 10 percent of keratinising tumours on the ears. A comparative study of 33 basosquamous, 1,796 basal cell, and 736 squamous cell carcinomas of the head and neck found that the basosquamous lesion has a potential to recur and metastasise similar to squamous cell lesions. Three cases of metastatic basosquamous carcinoma were reported in 1989, two of which were rapidly fatal, and the authors noted that diagnostic features may be absent in a small biopsy and that metastasis is usually associated with a poor prognosis. A 2004 case report described a 44-year-old woman with extensive ulcero-vegetative lesions involving the anterior half of the scalp, the left orbit, and the left side of the face, with basosquamous carcinoma infiltrating cerebral tissue.

Surgical treatments, including wide excision and Mohs' micrographic surgery, remain the treatment of choice according to a 2023 narrative review. The same review states that Hedgehog pathway inhibitors and checkpoint inhibitors must be thoroughly investigated with large controlled trials, as they may offer an alternative for irresectable or difficult-to-treat locally advanced cases. A 2024 survey of UK dermatologists found significant variability in disease categorisation, treatment approaches, and follow-up protocols, with a noted lack of standardised guidelines. The 2020 series concluded that basosquamous carcinoma should be considered a distinct type of keratinising tumour with different anatomical, sex, and age distributions compared to basal cell and squamous cell carcinomas.

The evidence is limited to small case series, a single comparative study of 33 cases, and narrative reviews. No randomised controlled trials exist for any treatment in basosquamous carcinoma. What is missing is prospective data, standardised diagnostic criteria, and large controlled trials of Hedgehog pathway inhibitors or checkpoint inhibitors. The 2024 UK survey underscores that no consensus management guidelines exist, and the 2023 review calls for thorough investigation of systemic therapies. Without these, treatment decisions remain based on extrapolation from basal cell and squamous cell carcinoma data rather than direct evidence for basosquamous carcinoma.

Cutaneous Basosquamous Carcinoma of the Head and Neck: A Comparative Analysis

AbstractConsiderable debate has taken place concerning cutaneous basosquamous carcinomas. Some authors believe they are merely a variant of basal cell carcinoma, based on the apparent rare occurrence of metastases. This comparative study of 33 cases of basosquamous, 1,796 cases of basal cell, and 736 cases of squamous carcinomas arising in the head and neck demonstrates that the basosquamous lesion has the potential to recur and to metastasize, which is similar to squamous cell lesions. An aggressive primary treatment program is recommended.

Basosquamous Carcinoma: Comprehensive Clinical and Histopathological Aspects, Novel Imaging Tools, and Therapeutic Approaches

AbstractBasosquamous carcinoma (BSC), an uncommon and aggressive nonmelanoma skin cancer exhibiting characteristics ranging from basal cell carcinoma (BCC) to squamous cell carcinoma (SCC), is a subject of controversy in terms of its classification, pathogenesis, histologic morphology, biologic behavior, prognosis, and management. This narrative review is based on an electronic search of English-language articles in PubMed that included the terms "basosquamous carcinoma" and/or "metatypical carcinoma of the skin" in their titles. The review aims to succinctly present and assess current data on the epidemiology, clinical presentation, dermoscopic, LC-OCT, and histopathologic characteristics, as well as the genetics and management of BSC, providing insight into this intriguing entity. As a conclusion, dermoscopy, deep incisional biopsies, and immunohistologic techniques should be applied in clinically suspicious lesions to achieve an early diagnosis and better prognosis of this tumor. Surgical treatments, including wide excision and Mohs' micrographic surgery, remain the treatment of choice. Finally, Hedgehog pathway inhibitors and checkpoint inhibitors, must be thoroughly investigated with large controlled trials, since they may offer an alternative solution to irresectable or difficult-to-treat locally advanced cases of basosquamous carcinoma.

Cutaneous basosquamous carcinoma infiltrating cerebral tissue

AbstractBasosquamous carcinoma of the skin is a rare malignancy with specific histopathological features of both basal cell carcinoma and squamous cell carcinoma. Some authors believe that basosquamous carcinoma is a variant of basal cell carcinoma, while others suggest that this tumour may behave more aggressively. We present a 44-year-old female patient who was diagnosed with a basosquamous carcinoma histopathologically. She had extensive ulcero-vegetative lesions, involving the anterior half of the scalp, the left orbit and the left side of the face. With this case we aim to emphasize the aggressive nature of basosquamous carcinoma and review the literature.

Characterization of Basosquamous Cell Carcinoma: A Distinct Type of Keratinizing Tumour

AbstractBasosquamous carcinoma is a rare clinical entity, which comprises 1.7-2.7% of all skin carcinomas. It is described as a basal cell carcinoma with features of squamous differentiation. To date, studies of the epidemiology of basosquamous carcinoma have been few and small in size. We report here the most extensive series of basosquamous carcinomas published to date, highlighting the differences between basosquamous carcinoma and other keratinizing tumours. Patients undergoing surgical excision for keratinizing tumours were enrolled in this study. Age, sex and tumour characteristics were recorded. A total of 1,519 squamous cell carcinomas, 288 basosquamous carcinomas and 4,235 basal cell carcinomas were collected. Basosquamous features were compared with those of basal cell and squamous cell carcinomas. For basosquamous carcinomas, 70.5% were located on the head and neck, particularly on the nose, forehead and cheeks, and represented almost 10% of the keratinizing tumours on the ears. Significant differences were found between basosquamous carcinoma and basal cell or squamous cell carcinomas. Basosquamous carcinoma should be considered a distinct type of keratinizing tumour with different anatomical, sex and age distributions.

Basosquamous carcinoma: a survey study on current management in the UK

AbstractBasosquamous carcinoma (BSC) is a rare, aggressive nonmelanoma skin cancer with overlapping features of basal cell carcinoma and squamous cell carcinoma, often requiring histopathological analysis for accurate diagnosis. Our study surveyed UK dermatologists on their management practices for BSC, revealing significant variability in disease categorization, treatment approaches and follow-up protocols, with a noted lack of standardized guidelines. The findings of this study underscore the need for prospective studies and the development of treatment guidelines to improve the management of BSC.
