What the evidence adds up to
A 2012 case report describes a 59-year-old African woman with gallstones and situs inversus totalis who underwent a single-port cholecystectomy using a single trocar access device. The procedure was uneventful, and the authors concluded that this approach may be an option for such surgery even in patients with situs inversus totalis. No other patients were reported, and no comparative data exist.
A 2014 case report describes a 57-year-old man with hepatocellular carcinoma who received a liver transplant from a 73-year-old woman with situs inversus totalis, a donor considered marginal. The liver was implanted using a piggyback technique, positioning the larger hemiliver in the left upper quadrant. The graft reperfused well without surgical or anaesthetic problems. The patient was discharged on postoperative day 7 and was alive and in good clinical condition at 8 months follow-up. The authors state this is the first reported use of the 1992-Belghiti piggyback technique with a situs inversus totalis donor graft and argue that donor situs inversus totalis should not be considered a contraindication for liver transplantation.

Single-port cholecystectomy in a patient with situs inversus totalis presenting with cholelithiasis: a case report
AbstractINTRODUCTION: Situs inversus totalis (mirror image organs) is a rare condition and may affect the intra-abdominal viscera as well as the intrathoracic organs. Cholelithiasis is not more common in these conditions, but the diagnosis may be more difficult. CASE PRESENTATION: We present the case of a 59-year-old African woman with gallstones and situs inversus totalis. A single-port cholecystectomy was performed using a single trocar access device (SITRACC). CONCLUSIONS: The procedure was uneventful, showing that this approach may be an option for this kind of surgery even in patients with situs inversus totalis.

Side-to-side caval anastomosis in a patient receiving a liver graft from a marginal donor with situs inversus totalis
AbstractINTRODUCTION: The donor situs inversus totalis status was considered an absolute contraindication to liver transplantation due to the technical difficulties involved. Only in recent years has a very young deceased donor with situs inversus totalis been considered as a potential donor. PRESENTATION OF CASE: We herein report a single case of 57-year-old male patient with hepatocellular carcinoma who received a liver transplantation from a 73-year-old woman with situs inversus totalis. Liver was implanted using a 1992-Belghiti piggyback technique positioning the larger hemiliver in the left upper quadrant and the left in the liver fossa. We assisted a good graft reperfusion without surgical or anesthetic problems. His hospital stay was relatively uneventful and he was discharged from hospital on postoperative day 7. At 8 months of follow-up the patient is alive and in good clinical condition. DISCUSSION: The donor situs inversus totalis does not require any modification of transplant procedure if the donor-recipient size match permits a comfortable placement of the graft in a standard anatomical position. To the best of our knowledge, this is the first case of liver transplantation with a graft from a "marginal" donor with situs inversus totalis using a 1992-Belghiti piggyback technique. CONCLUSION: The donor situs inversus totalis status should not be considered a contraindication for LT and the piggyback technique should be considered the surgery of choice.

AbstractSitus inversus totalis is a rare syndrome, with an estimated prevalence of 1/10,000 births, characterized by the inverted position of the thoracic and abdominal organs with respect to the sagittal plane. In a patient with situs inversus totalis the anaesthetic management during the respective surgical procedure is challenging. The present case report lays an emphasis on the potential difficulties during anaesthetic management and its various implications.
