Cefaclor vs amoxicillin in the treatment of acute, recurrent, and chronic sinusitis
AbstractBACKGROUND: The treatment of acute, recurrent, and chronic sinusitis remains controversial because of the presence of a wide variety of aerobic and anaerobic bacteria in the sinuses. DESIGN: This double-blind, randomized trial compared cefaclor with amoxicillin in the treatment of acute, recurrent, and chronic maxillary sinusitis using clinical evaluation, roentgenography, and microbiologic evaluation of antral aspirates. SETTING: Outpatient office of five otorhinolaryngologists in Salt Lake City, Utah. PATIENTS: One hundred eight adult patients with acute, recurrent, or chronic maxillary sinusitis. INTERVENTION: Oral treatment with cefaclor (500 mg) twice daily or amoxicillin (500 mg) three times daily for 10 days. MAIN OUTCOME MEASURE: Clinical response to treatment with cefaclor vs amoxicillin. RESULTS: Fifty-six patients with acute sinusitis, 25 with recurrent sinusitis, and 15 with chronic sinusitis were evaluable. Although multiple organisms were common in each group, patients with acute sinusitis were more likely to have Haemophilus influenzae or Streptococcus pneumoniae, and patients with recurrent or chronic sinusitis were more likely to have anaerobes in sinus aspirate. Whether treated with cefaclor or amoxicillin, clinical improvement occurred in 86% of patients with acute sinusitis and 56% of patients with recurrent sinusitis. Patients with chronic sinusitis were too few to allow statistical analysis of the differences in outcome between them and patients with recurrent or acute sinusitis. Resistance of the cultured organisms to the study drug used was unrelated to treatment outcome. CONCLUSIONS: The rate of clinical improvement was high in patients with acute sinusitis but was less favorable in those with recurrent and chronic disease regardless of the study drug used. The susceptibility of organisms isolated to the study drugs was unrelated to outcome.

Medical Therapies for Adult Chronic Sinusitis: A Systematic Review
AbstractChronic sinusitis is an inflammatory disease of the paranasal sinuses that usually manifests as chronic sinonasal symptoms.. Since the pathophysiology of chronic rhinosinusitis in children and adults varies significantly, as do the medical and surgical treatments, this review exclusively discusses adult chronic rhinosinusitis and explores the medical therapies used to treat chronic sinusitis in adult patients. A systematic review was performed involving studies of medical therapies used to treat chronic sinusitis in adult patients between 2016 and 2022. The PubMed and Google scholar databases were used to explore studies regarding our subject. The keywords included "Medical therapy, treatment, chronic, sinusitis, rhinosinusitis, and adult" and were used in various combinations. Original research reporting medical therapies used in treating chronic sinusitis in adult patients and full-text publications served as the inclusion criterion. Though 166 articles were obtained, only eight met the inclusion criteria. The studies included 2884 participants; all the studies were clinical trials. The review found Dupilumab to be well-tolerated and provided rapid, significant, and clinically meaningful improvements for patients with chronic sinusitis and nasal polyposis (CRSwNP). In addition, it produced rapid and sustained improvement in the sense of smell, alleviating a cardinal symptom of severe CRSwNP. Furthermore, Dupilumab improved upper and lower airway outcome measures and health-related quality of life in patients with severe CRSwNP and comorbid asthma. Another therapy was Omalizumab, which produced positive outcomes on all endpoints in participants who switched from placebo to Omalizumab through week 52. In addition, endoscopic sinus surgery, combined with medical therapy, it was found to be more efficacious than medical therapy alone in patients with CRSwNP.

AbstractSymptoms of sinusitis are among the most common reasons for patients presenting to primary care physicians. There is considerable controversy regarding appropriate management of both acute and chronic sinus disease. This article reviews etiologic mechanisms in these conditions and presents recent evidence to provide a logical basis on which to make decisions regarding medical and surgical therapy.
AbstractSinusitis is an exceptionally common disorder, affecting an estimated 35 million Americans per year. The development of sinusitis requires both the presence of a virulent pathogen and obstruction of the normal drainage pattern from the sinus cavities. Moreover, the development of chronic or recurrent sinusitis indicates a failure of the local immune system to prevent or effectively combat the infection. Identification of the components of the upper respiratory immune defense system and the possible areas of dysfunction that predispose to sinusitis may be important steps in eventually preventing this common disease.

Pathogenic Bacteria Associated with Sinusitis: A Review Article
AbstractThe sinusitis, an inflammation of paranasal sinuses, is one of the most common diseases that affect many individuals all over the world. The etiology and epidemiology of sinusitis result from complex multifactor interaction of environmental, genetic and infectious factors. Management typically involves both pharmacological and non-pharmacological means. The rationale being, sinusitis, an inflammation of the sinus cavities, is usually a disorder perpetuated by pathogenic bacteria. It is critical to have a complete understanding of the microbial landscape associated with the disease to develop proper therapeutic regimens. In this literature review, recent studies are synthesized about the contribution of pathogenic bacteria to sinusitis, wherein interrelations between different microbial populations and host factors are also proposed as gaps for future research. In addition, treatment and therapeutic strategies have been also highlighted.

AbstractChronic sinusitis with polyps is a complex disease with suspected contribution of multiple genetic and environmental factors. The search for causative genes has led to the discovery of numerous candidates. Pathways analysis applied to these candidate genes identified common central molecules that are likely to be key mediators of the disease process. Novel therapies targeting these molecules may be applicable for the treatment of chronic sinusitis with polyps.
