What the evidence adds up to

Sinonasal undifferentiated carcinoma is a rare and extremely malignant tumour of the paranasal sinuses that typically presents at a late stage with poor overall survival. A 2024 case report describes a 75-year-old man with frontal base and orbital involvement whose tumour progressed 12 months after surgery despite adjuvant chemotherapy and 30 cycles of radiotherapy. The same report states that different combinations of chemotherapy, radiotherapy and radical surgical resection have been used but there is still no universal treatment strategy. A 2018 update on survival and prognostic markers notes that there are no agreed guidelines for treatment and few studies showing survival benefits for any treatment modality.

Multiple reviews from 2005, 2004, and 2018 conclude that an aggressive multimodality approach combining surgery, platinum-based chemotherapy, and radiation seems to offer the greatest chance for locoregional control and survival. The 2005 review specifies that a chemoradiotherapy regimen including concurrent platinum-based chemotherapy given preoperatively or postoperatively to patients with resectable disease appears to result in better disease-free survival. However, the 2004 systematic review states that numbers in individual studies are too small to produce a definitive opinion on the standard of care. The 2018 update on survival and prognostic markers presents data from 40 patients treated between 1995 and 2017, comparing survival outcomes by treatment modality and assessing clinical factors associated with survival less than three years, but does not report a specific survival rate or response rate in the abstract.

The 2023 study analysed SNUC cases from the past ten years for overall survival and disease-free survival and performed a systematic literature research, but its abstract gives no numerical results. The 2024 case report notes that slight disease progression was seen 12 months after surgery and the patient remained under regular adjuvant treatment. Across all abstracts, outcomes are described as poor, and the disease is characterised by rapid growth, bony destruction, and advanced stage at presentation.

What is still missing are prospective trials with sufficient patient numbers to establish a standard treatment sequence, reliable biomarkers or molecular stratification to guide therapy, and dedicated funding for a disease so rare that no randomised controlled trials exist.

Sinonasal undifferentiated carcinoma: an update

AbstractPURPOSE OF REVIEW: Sinonasal undifferentiated carcinoma is a rare and extremely malignant tumor of the paranasal sinuses. Historically, treatment outcomes have been poor. This review presents recent data on the management of sinonasal undifferentiated carcinoma and examines treatment trends that may result in improved locoregional control and survival. RECENT FINDINGS: Patients who receive aggressive multimodality treatments have improved outcomes. In particular, a chemoradiotherapy regimen including concurrent platinum-based chemotherapy given preoperatively or postoperatively to patients with resectable disease seems to result in better disease-free survival. Neoadjuvant chemotherapy, although strongly advocated by some, is currently not offered by many. Although surgery seems to be an important part of the treatment for sinonasal undifferentiated carcinoma, its ideal timing, either upfront or after radiotherapy, remains uncertain. SUMMARY: Overall, outcomes for sinonasal undifferentiated carcinoma are poor. An aggressive approach using surgery, platinum-based chemotherapy, and radiation seems to offer the greatest chance for significant locoregional control and survival.

Sinonasal Undifferentiated Carcinoma: Case Series and Systematic Review of the Literature

AbstractSinonasal undifferentiated carcinoma is a rare, highly aggressive malignancy. A number of case series have been published in the literature. Most authors recommend aggressive management with a combination of surgery, radiotherapy, and chemotherapy, but the numbers in the individual studies are too small to produce a definitive opinion on the standard of care. In an attempt to determine the optimal treatment for this condition, we have undertaken a systematic review of the literature to evaluate all cases of sinonasal undifferentiated carcinoma that have been published since its initial description in 1986. Patient demographics, extent of the tumour at presentation, management, and outcomes were evaluated. We also present the experience from our institution.

Oncologic management of sinonasal undifferentiated carcinoma

AbstractPURPOSE OF REVIEW: This article reviews the latest treatment paradigms in sinonasal undifferentiated carcinoma (SNUC). RECENT FINDINGS: The aggressive biology and associated advanced presentation of SNUC make successful treatment a challenge shared across medical specialties. Still, studies reporting outcomes in SNUC indicate that an aggressive treatment strategy consisting of surgery, radiation and chemotherapy offers the best chance of prolonged survival. SUMMARY: Successful treatment of SNUC requires highly specialized care at tertiary cancer treatment facilities. A better understanding of the biology of the disease coupled with increasing outcome reporting will lead to optimized treatment regimens.

Sinonasal Undifferentiated Carcinoma: An Update on Survival and Prognostic Markers

AbstractSinonasal undifferentiated carcinoma (SNUC) is a rare tumor most often arising from the sinonasal cavity and presenting at a late stage with poor overall survival. There are no agreed upon guidelines for treatment and there are few studies showing survival benefits for any treatment modality. We have previously presented data from 19 patients from 1995 to 2008. Here, we present follow-up data from our institution for 21 additional patients (total of 40) treated between 1995 and 2017. The aims of this study were to compare survival outcomes based on treatment modality and to assess for clinical factors associated with poor patient outcomes (defined as survival < 3 years).

AB018. It’s not always olfactory neuroblastoma—sinonasal undifferentiated carcinoma with frontal base and orbital involvement, a rare disease with poor prognosis: a case report

AbstractBACKGROUND: Malignant neoplasms of the paranasal sinuses and nasal cavity are rare, comprising only 3% of all head and neck malignancies. Sinonasal undifferentiated carcinoma is a rare malignancy and an aggressive neoplasm that was clinic-pathologically distinct from other poorly differentiated malignancies of the nasal cavity and sinuses. The lesion is thought to originate from the epithelium, grows rapidly and invades nearby structures, often resulting in bony destruction while rapid progression of symptoms over weeks to months is characteristic. CASE DESCRIPTION: A 75-year-old man initially presented to ophthalmologist due to blurry vision and diplopia for 2 months. There was also progressive right eye swelling and pain for the past 12 months. Further image study revealed a right superomedial orbital mass with frontal/ethmoid sinus and frontal base extension, causing significant right eye gaze limitation, visual loss, ptosis, chemosis and exophthalmos. He was then transferred to neurosurgical clinic for further treatment where combined surgery was advocated to relieve the mass effect and also for pathology proof. Orbital roof/medial wall and frontal skull base were invaded and destroyed by the tumor. A bicoronal craniotomy was performed and extensive tumor removal was achieved except the orbital fossa due to difficult separation from the ocular muscle and optic nerve. Frontal base was reconstructed in a layered fashion to avoid cerebrospinal fluid (CSF) leakage and infection. He was sent to intensive care unit (ICU) for overnight observation where no further neurologic deterioration occurred. Instant orbital pain relief was noted with significantly decreased proptosis and improving visual acuity. Detailed pathological staining and molecular tests revealed a sinonasal undifferentiated carcinoma, adjuvant chemotherapy was arranged, followed by 30 cycles of radiotherapy. Slight disease progression was noted 12 months after the surgery, chemotherapy regimen was adjusted according to clinical response and he is still currently under regular adjuvant treatment. CONCLUSIONS: Sinonasal undifferentiated carcinoma typically carries a poor prognosis despite aggressive medical and surgical treatment, as it always presents at an advanced stage. Different combinations of chemotherapy, radiotherapy, radical surgical resection have been used to improve the outcome yet there is still not a universal treatment strategy. Early diagnosis, prompt treatment, and comprehensive medical care are crucial to achieve the best possible outcome for affected patients.
