What the evidence adds up to
A 1999 rural population-based case-control study in India enrolled 258 cases and 308 controls from one centre and 301 cases and 591 controls from another, all aged 40–60 years. Systolic blood pressure (odds ratio 1.4 and 1.5 across centres) and daily duration of sunlight exposure (odds ratio 1.6 and 1.4) were associated with senile cataract. Other associations reaching significance in at least one centre included family history of cataract (odds ratio 5.0), duration of exposure to fire and dust per day (odds ratio 1.8), use of cheap cooking fuels (odds ratio 1.8), utilisation of rice gruel (odds ratio 0.5), increased height (odds ratio 0.7), and increased hours of work per day (odds ratio 0.7). The authors concluded that senile cataract has a multifactorial aetiology and that further studies are needed before preventive measures can be initiated.
A 2017 cohort study of 22,377 nuclear industry workers (25.4% female, mean age at first employment 24.9 years) followed from 1948 to 2008 examined the relative risk of senile cataract in relation to occupational external gamma-ray exposure. Mean cumulative gamma-ray dose was 0.54 Sv in males and 0.44 Sv in females. The relative risk of senile cataract increased with attained age and was significantly higher in workers diagnosed with glaucoma or high myopia. Risk increased with external gamma-ray dose and was highest in workers who had received cumulative doses above 2.00 Sv. No dependency on sex, age at initial employment, smoking, alcohol consumption, or diabetes mellitus was found. The authors concluded that incidence depended on both non-radiation and radiation factors.
A 1988 German review article argued that the term "senile cataract" covers a multiplicity of morphologically different types of opacities with distinct properties. The authors stated that for clinical relevance and research it is essential to replace the term "senile cataract" with more precise definitions based on type and localisation of the opacity.
What is still missing is a unified, subtype-specific classification that would allow trials to stratify patients by opacity type, and the funding to conduct prospective studies testing whether modifying the environmental and occupational risk factors identified in these observational studies can actually delay or prevent cataract formation. No drug is mentioned in any of these abstracts.

A Rural Population Based Case-Control Study of Senile Cataract in India
AbstractPURPOSE: Senile cataract contributes to 75% of blindness in India and there is a growing backlog of cataract cases needing surgery. The present study seeks clues to the etiology of senile cataract, so that strategies to prevent or even delay cataract formation could be planned. METHODS: Using a community based case-control design, 258 cases & 308 controls from one centre and 301 cases & 591 controls from another were studied. The subjects were from rural areas and were aged 40-60 years. Logistic regression analysis technique was employed to study the associations between senile cataract and various variables. RESULTS: Systolic blood pressure, duration of exposure to sunlight per day were associated with senile cataract in both the centres (OR = 1.4 & 1.5 for systolic BP and 1.6 & 1.4 for exposure to sunlight). Utilization of rice gruel (OR = 0.5), duration of exposure to fire & dust per day (OR = 1.8), family history of cataract (OR = 5.0), use of cheap cooking fuels (OR = 1.8), increased height (OR = 0.7) and increased number of hours of work per day (OR = 0.7) were other variables that showed significant association in either of the centres. CONCLUSION: Senile cataract appears to have a multi factorial etiology. Though the study provided some clues to the etiology of senile cataract, further studies are needed to know the specific role of these factors in the causation of cataract, so that any preventive or control measures could be initiated in the community. Till such time, we have to fall back on the available surgical approach in control of senile cataract.

Risk of senile cataract among nuclear industry workers
AbstractAIM: to assess the relative risk (RR) of senile cataract in a cohort of workers occupationally exposed to external gamma-rays over a long time period. MATERIAL AND METHODS: The RR of cataract development was studied in a cohort of nuclear workers that were first employed in 1948-1982 and followed up till the end of 2008 (n=22,377; females, 25.4%). The mean age at initial employment was 24.9 years. The mean cumulative gamma-ray dose was 0.54 Sv in males and 0.44 Sv in females. The analysis performed with EPICURE software provided RR estimates for cataract incidence in relation to a number of non-radiation and radiation factors. RESULTS: The RR of senile cataract increased with the attained age of the workers. The RR was significantly higher in males during the 2006-2008 period as compared to the 1996-2005 period. The RR was significantly higher in workers that were diagnosed with glaucoma and high myopia as compared to those that were not. The risk of senile cataract increased with the external gamma-ray dose and was the highest in workers who had formerly been exposed to doses above 2.00 Sv. The RR of senile cataract showed no dependency on sex, age at initial employment, smoking status, alcohol consumption, or diabetes mellitus. CONCLUSION: The incidence of senile cataract in the cohort under study was found to depend on both non-radiation and radiation factors.

AbstractIn this article the authors review the present state of our knowledge concerning senile cataract. Clinical observations, frequencies and prognosis, biochemical and biometrical results, etiology and pathogenesis as well as aspects of cataract epidemiology are presented. The available information indicates that the term "senile cataract" comprises a multiplicity of morphologically different types of opacities all characterized by unique properties. With respect to clinical relevance and for research purposes it is obvious and essential to replace the term "senile cataract" with more precise definitions appropriate to the type and localization of the opacity.
