What the evidence adds up to

A 2018 study of 204 Korean probands with schizophrenia and their families (543 family members total) found that seven of ten MMPI personality dimensions were significantly heritable. The three groups—controls, unaffected first-degree relatives, and cases—differed significantly in the expected order of average scores for all heritable dimensions. The authors concluded that aberrations in several personality dimensions could form part of the complexity of schizophrenic syndrome through genetic–environment interactions, but noted limitations in family recruitment and phenotyping.

A 2023 systematic review of case reports and case series on schizoid personality disorder described the condition as characterised by lack of interest in social activities, difficulty expressing emotions, and a preference for being alone. The review stated that professionals believe the disorder is likely caused by genetic, social, and environmental factors. It did not report any quantitative outcomes, response rates, or survival data.

A 2000 review of the biological basis of schizotypal personality disorder noted that schizotypal patients share social isolation, cognitive impairment, imaging and biochemical evidence of brain dysfunction, and genetic similarities with schizophrenic patients, but do not become chronically psychotic. The authors argued that studying these patients could clarify genetic and biological factors in the schizophrenia spectrum and help identify protective factors against florid psychosis.

No drug treatment is mentioned in any of these abstracts. What is missing is any randomised controlled trial of a pharmacological intervention for schizoid personality disorder, any validated biomarker to stratify patients, and any funding for such a trial.

Heritability and Familiality of MMPI Personality Dimensions in the Korean Families with Schizophrenia

AbstractOBJECTIVE: Categorical syndrome such as schizophrenia could be the complex of many continuous mental structure phenotypes including several personality development/degeneration dimensions. This is the study to search heritability and familiality of MMPI personality dimensions in the Korean schizophrenic LD (Linkage Disequilibrium) families. METHODS: We have recruited 204 probands (with schizophrenia) with their parents and siblings whenever possible. We have used MMPI questionnaires for measuring personality and symptomatic dimensions. Heritabilities of personality dimensions in total 543 family members were estimated using Sequential Oligogenic Linkage Analysis Routines (SOLAR). Personality dimensions in total family members were compared with those in 307 healthy unrelated controls for measuring the familialities using ANOVA analysis. RESULTS: Seven of the 10 MMPI variables were significantly heritable and were included in the subsequent analyses. The three groups (control, unaffected 1st degree relative, case) were found to be significantly different with the expected order of average group scores for all heritable dimensions. CONCLUSION: Our results show that the aberrations in several personality dimensions could form the complexity of schizophrenic syndrome as a result of genetic-environment coactions or interactions in spite of some limitations (recruited family, phenotyping).

The biological basis of schizotypal personality disorder

AbstractAbstract Schizotypal personality disorder is the DSM equivalent of the ICD category schizotypal disorder. It may be seen as part of the schizophrenia spectrum of disorders. Patients with schizotypal personality disorder (SPD) share many features with schizophrenic patients. These include symptoms of social isolation and cognitive impairment. They also share imaging and biochemical evidence of brain dysfunction, and genetic similarities. SPD patients, however, for some reason do not become chronically psychotic. The study of schizotypal patients is of great importance in attempting to clarify the genetic and biological basis of the schizophrenia spectrum. It is also important in helping to delineate the differences between schizophrenic and schizotypal patients, and to distinguish those factors that protect the schizotypal patient from becoming floridly psychotic. In this article we present recent findings regarding the biological basis of schizotypal personality disorder. We illustrate the contribution of these findings to our understanding of risk and protective factors in the schizophrenia spectrum.
