What the evidence adds up to

Salivary gland diseases encompass a wide range of inflammatory, infectious, and neoplastic conditions that can significantly impair quality of life when salivary flow is reduced. A 2009 review notes that the functions of saliva are often taken for granted until dysfunction occurs, and outlines common clinical entities and practical treatment guidelines. A 1987 paper similarly catalogues oral complications of salivary gland dysfunction and discusses palliative and therapeutic regimens, though neither paper reports quantitative outcomes from specific interventions.

For malignant salivary gland tumours, which represent less than 1% of all malignancies, systemic therapy is standard for locoregional recurrent or metastatic disease. A 2011 review of cytotoxic agents from small phase II studies reports objective response rates ranging from 15% to 50%, with duration of response typically between 6 and 9 months. The same review states that these modest results make evaluation of novel therapies mandatory, and recommends that only patients with progressive disease be enrolled in trials, with stringent inclusion criteria to accurately report response and stabilisation. A 2008 case report describes a 72-year-old woman with a small cell carcinoma of the parotid gland who received radiotherapy because surgery was not possible; the authors note that first-choice therapy for major salivary gland tumours is surgery combined with radiation or chemotherapy, and that primary radiotherapy with or without chemotherapy is an alternative when surgery is not feasible.

A 2018 retrospective review of a dedicated salivary gland centre over ten years reports that gland removal, previously the treatment of choice for most benign salivary gland diseases, has been largely replaced by gland-sparing procedures, and that surgery-related morbidity has been markedly reduced. A 2019 systematic review on salivary biomarkers in systemic diseases notes that many systemic conditions affect salivary gland function and composition, but that the use of salivary variables as diagnostic markers remains limited. What is still missing are adequately powered prospective trials for malignant disease that can improve on the modest 15–50% response rates and 6–9 month duration of response seen with current cytotoxic agents, as well as validated diagnostic biomarkers for systemic diseases affecting the glands.

Role of irradiation in the suppression of parotid secretions

AbstractSuppression of salivary flow is of value in the management of salivary fistulae and sialectasia. It may also be beneficial in mentally defective patients and those with neurological palsies lacking control of their salivation. Nine patients were treated by irradiation to the parotid gland to control salivary flow; eight had complete resolution of symptoms and one had partial relief. Irradiation was effective as primary treatment and after failed surgery and/or drug treatment. Low doses were effective and there were no significant acute or long-term side effects. Its use avoids long-term medication and their potential side effects and may reduce the need for surgery.

Management of Oral Sequelae

AbstractThe purpose of this paper is to enumerate and describe the oral complications that are associated with salivary gland dysfunction. The significance of these oral problems and their related adverse impact on the patient's quality of life are presented. Palliative and therapeutic agents and regimens are discussed, and a suggested protocol for managing the deleterious oral sequelae of salivary gland dysfunction is outlined.

Salivary Gland Disease

AbstractSaliva serves a variety of important functions, many of which are not appreciated until salivary flow decreases. A wide spectrum of pathologic conditions, including inflammatory, infectious, and neoplastic, affect the major and minor salivary glands, resulting in potentially significant morbidity and diminished quality of life for the patient. This chapter provides a review of conditions affecting the salivary glands, with attention to the more common entities that are likely to be encountered in clinical practice along with relevant practical treatment guidelines.

Management of salivary gland malignancies: current and developing therapies

AbstractSalivary gland tumors are rare, clinically diverse neoplasms that represent less than 1% of all malignancies. In locoregional recurrent or metastatic disease, systemic therapy is the standard approach. While numerous small phase II studies have evaluated the activity of cytotoxic agents, either alone or in combination, the response rates are generally modest with objective response rates ranging from 15%–50%. Duration of response is cited in the range of 6–9 months. Given this, further evaluation of novel therapies is mandatory in these diseases. With the emergence of molecular targeted therapy, these tumors become optimal candidates for trials of investigational drugs and established drugs for new indications. Of note, given the often indolent nature of disease, only patients with progressive disease should be enrolled and treated on these clinical trials. Study designs must incorporate stringent inclusion criteria to enable accurate reporting of disease response and stabilization. With dedication and co-operation, patients with these rare neoplasms can be accrued to clinical trials and the establishment of new treatment guidelines will be forthcoming.

AbstractBACKGROUND: Small cell carcinoma of the major salivary glands are very seldom. If there is such a tumor always have in mind that it can be a metastatic process of another small cell carcinoma in other locations of the body. That's why first of all a primary in the bronchial system has to be excluded with special diagnostics. CASE REPORT: We present the case of a 72-year-old woman suffering from a small cell carcinoma of Parotid gland. Because a surgical therapy was impossible she underwent a radiotherapy. CONCLUSION: First choice for therapy of tumors of the major salivary glands is surgical therapy in combination with radiation/chemotherapy. If this is not possible a primary radiotherapy sometimes in combination with chemotherapy seems to be another therapeutic option.

AbstractSalivary diagnosis is an increasingly important field in dentistry, physiology, internal medicine, endocrinology, pediatrics, immunology, clinical pathology, forensic medicine, psychology, and sports medicine. A growing number of drugs, hormones, and antibodies can be reliably monitored in saliva, which is an easily obtainable, non-invasive diagnostic medium It has become apparent that many systemic diseases affect salivary gland function and salivary composition. Studies of the effects of systemic diseases on salivary variables have been valuable in understanding the pathogenesis of the diseases, but their use as diagnostic markers has been limited. This article aims to review the point in the literature role of salivary biomarkers in systemic diseases.

Developments in a dedicated salivary gland centre: a 10‐year retrospective review

AbstractAbstract Diseases of the salivary glands are uncommon and patients present to a number of specialties. The result is that experience is thinly spread across clinical specialities. A dedicated salivary gland clinic was set up at Guy's Hospital London fifteen years ago. The profile of patients referred to the centre over the last ten years was analysed and changes in management mapped over time with respect to methods of investigation and treatment. Gland removal, which was previously the treatment of choice for most benign salivary gland diseases has been replaced by gland sparing procedures which have rendered gland excision to a minor role in management. Salivary gland surgery‐related morbidity has been markedly reduced.

Salivary glands

AbstractRecent developments in the management of salivary gland disease include confirmation of the diagnostic role of fine needle aspiration of salivary gland masses. Other investigations such as computerized tomography sialography and positron emission tomography also hold great potential. The relatively recently recognized clinical entity of salivary duct carcinoma is associated with a poor prognosis, particularly in its infiltrating form, whereas treatment results for early stage adenoid cystic carcinoma are surprisingly good. Developments in HIV-related salivary gland disease continue to evolve. Of particular interest are the diffuse infiltrative lymphocytosis syndrome and cystic lymphoepithelial lesions of the salivary glands. The role of pilocarpine in the management of postirradiation xerostomia and the importance of a multidisciplinary approach to the management of the drooling patient are also discussed in this review.
