What the evidence adds up to
A retrospective review of 73 patients with major salivary gland carcinoma ex pleomorphic adenoma treated between 1960 and 1994 found that 55% died of the disease. Among 66 patients with primary tumours, 23% had local recurrence; regional metastasis occurred in 56% and distant metastasis in 44%. Overall survival was 39% at three years and 30% at five years. All patients were treated surgically, and 32 also received radiation therapy. Adenocarcinoma (31 cases) and salivary duct carcinoma (24 cases) were the most common malignant subtypes. The authors concluded that tumour size, grade, and clinical and pathologic stage were important prognostic factors, and that patients with minimally invasive tumours (less than 5 mm) should do well with appropriate surgical treatment.
A separate case report describes a clear cell carcinoma arising in a pleomorphic adenoma of a minor salivary gland, noting that malignant change in pleomorphic adenoma is uncommon, with a reported incidence of 2–10 per cent. Another report presents a single case of carcinoma ex pleomorphic adenoma of the upper lip in a 52-year-old woman, stating that this tumour comprises less than 4% of salivary gland neoplasms and 11.6% of salivary gland carcinomas, and that occurrence in minor salivary glands is rare.
One abstract reviews signalling pathways in salivary gland adenoid cystic carcinoma, a different tumour type, and does not address carcinoma ex pleomorphic adenoma. No drug treatments are mentioned in any of the abstracts.
What is still missing are prospective trials, any data on systemic therapy, and molecular stratification that might identify which patients with invasive disease could benefit from treatments beyond surgery and radiation.

Carcinoma ex pleomorphic adenoma: A clinicopathologic review
AbstractBACKGROUND: Carcinoma ex pleomorphic adenoma (CXPA) is an aggressive, poorly understood salivary gland malignancy. Misdiagnosis is common, because the residual mixed tumor component may be small, and various carcinoma subtypes are possible. METHODS: We retrospectively reviewed the medical records of 73 patients with major salivary gland CXPA treated at our institution from 1960 to 1994. Of the 73 patients, 66 had primary tumors and 7 had recurrent tumors; 47 were men, and 26 were women; the mean age was 61 years. RESULTS: Adenocarcinoma (31 cases) and salivary duct carcinoma (24 cases) were the most common malignant subtypes. All patients were treated surgically, and 32 also had radiation therapy. Of 66 patients with primary tumors, 23% had local recurrence. Metastasis (either initial or delayed) occurred regionally in 56% and distantly in 44%. Thirty-six patients (55%) died of the disease. At 3 years, overall survival was 39% and at 5 years, 30%. CONCLUSIONS: Important prognostic factors include tumor size, grade, and clinical and pathologic stage. Patients with minimally invasive tumors (<5 mm) should do well with appropriate surgical treatment.

Clear cell carcinoma arising in pleomorphic adenoma of the minor salivary gland
AbstractMalignant change arising in pleomorphic adenoma of the salivary glands is uncommon, with a reported incidence of 2-10 per cent. Only one case where a pleomorphic adenoma became a clear cell carcinoma appears to have been published in the English literature. An additional case of clear cell carcinoma arising in pleomorphic adenoma of a minor salivary gland is reported and the relevant literature discussed.

Carcinoma Ex Pleomorphic Adenoma of Upper Lip: An Unusual Presentation
AbstractABSTRACT Carcinoma ex-pleomorphic adenoma is a rare salivary gland malignancy, usually derived from a long-standing or recurrent benign tumor, the pleomorphic adenoma. 1 This rare tumor comprises less than 4% of salivary gland neoplasms and 11.6% of salivary gland carcinomas. They mostly involve the “major” salivary glands 1 and their occurrence in the minor salivary gland is considered rare, especially in the upper lip. So far there is only one report of CXPA occurring in the upper lip in the literature. 13 Hereby, we present a case of carcinoma ex-pleomorphic adenoma in a 52-year-old woman who reported to our department with a painful swelling on the upper lip which was diagnosed and treated successfully.
A case of carcinoma ex pleomorphic adenoma occurred in the sublingual gland
AbstractAbstract Carcinoma ex pleomorphic adenoma is a malignant salivary gland mixed tumor arising from an existing pleomorphic adenoma (PA). Less than 1% of cases occur in the sublingual gland. Salivary gland duct carcinomas (SDCs) also exhibit invasive development and are rarely found as a microinvasive lesion. A 47‐year‐old man complained of swelling of the oral floor during general dental treatment and was referred to our department. Excision was performed, and we histopathologically determined an early stage of SDC ex PA confirmed in the PA. Tumor from sublingual gland should be carefully treated in consideration of malignancy.
