What the evidence adds up to
A 1996 review of chemotherapy for metastatic adenoid cystic carcinoma of salivary glands found no single drug or combination that could be regarded as a treatment of choice. Among four unpublished cases given cyclophosphamide, one patient responded and another obtained pain relief. The authors concluded that systematic, multi-institutional studies were needed to determine appropriate systemic treatment.
A 2017 case report described a 50‑year‑old woman with a minor salivary gland adenoid cystic carcinoma of the hard palate that was identified incidentally. After surgical treatment, no recurrence was noted at ten years of follow‑up. The report emphasised that adenoid cystic carcinoma is a slow‑growing but aggressive neoplasm with a tendency for late recurrence, and that the surgeon’s clinical judgement should guide treatment.
A 2014 review of signalling pathways stated that invasion and metastasis of salivary gland adenoid cystic carcinoma involve multiple signalling pathways and complex mechanisms, but it did not report any clinical data or therapeutic interventions.
The evidence base remains limited. No randomised trial has established a standard systemic therapy for metastatic disease. The single‑agent cyclophosphamide data from 1996 are sparse and decades old. What is missing is a properly funded, multi‑institutional trial that stratifies patients by tumour stage, histological subtype and molecular profile, and that tests prospectively defined drug regimens.

Metastatic Adenoid Cystic Carcinoma of Salivary Glands: Case Reports and Review of the Literature
AbstractBACKGROUND: Adenoid cystic carcinoma is an uncommon tumor of the salivary gland. Little has been published on the chemotherapy of this neoplasm. METHODS: The literature on this disease is reviewed, and data from seven unpublished cases are presented. RESULTS: Four patients received cyclophosphamide. One responded, and another had pain relief. The literature review did not identify any single drug or combination that might be regarded as the treatment of choice. CONCLUSIONS: Systematic, multi-institutional studies are required to determine appropriate systemic treatment for metastatic adenoid cystic carcinoma of salivary gland origin.

Coincidentally Identified Case of Minor Salivary Gland Adenoid Cystic Carcinoma
AbstractMinor salivary gland tumors of the oral cavity are rare. They represent approximately 29% of all minr salivary gland tumors and the most common intraoral site is hard palate. It is a slow growing but aggressive neoplasm with a tendency for recurrence. In this study we will present a 50 years old female patient who was presented with a mass localized at the right portion of the hard palate and was diagnosed incidentally with Adenoid Cystic Carcinoma. In the follow up, no recurrence was noted 10 years after the surgical treatment. Adenoid cystic carcinoma is a salivary gland malignancy with unexpected growth and late recurrence. Our aim was to review the treatment choices of the Adenoid Cystic Carcinoma under the light of current data and to express the importance of surgeon's clinical opinion upon the treatment to be applied.
