What the evidence adds up to
A 2013 review of retinal detachment surgery notes that oral fluoroquinolones may be associated with onset of retinal detachment, but the clinical relevance of this correlation is uncertain and the finding has not been replicated in subsequent studies. Pars plana vitrectomy continues to show efficacy as a primary treatment, especially in pseudophakic patients. For macula-on retinal detachment, scheduling surgery after a short time delay is not necessarily deleterious and may be beneficial. Bioerodible scleral buckling materials and artificial vitreous substitutes are under investigation.
A 1933 paper states that the function of a reattached retina is almost as complex a problem as the function of the normal retina, and that considerable work by physiologists, psychologists and ophthalmologists will be required to define how well a reattached retina functions after it has been detached for some time. The author notes that in cases of partial but progressive detachment, even an anatomic cure or reattachment without any return of function is a gain. AbstractPURPOSE OF REVIEW: Update on controversies in the surgical management of rhegmatogenous retinal detachment. RECENT FINDINGS: There are multiple new reports regarding the development and management of retinal detachment. Current use of oral fluoroquinolones may be associated with onset of retinal detachment, although the clinical relevance of this correlation is uncertain at this time and the finding has not been replicated in subsequent studies. Pars plana vitrectomy (PPV) continues to demonstrate efficacy as a primary treatment for retinal detachment, especially in pseudophakic patients. In many patients with macula-on retinal detachment, scheduling surgery after a short time delay is not necessarily deleterious and may actually be beneficial. Novel surgical tools, including bioerodible scleral buckling materials and artificial vitreous substitutes, are being investigated. SUMMARY: Retinal detachment remains an important cause of visual loss. Although current surgical techniques demonstrate high rates of anatomic and visual success, further advances will probably benefit patients with retinal detachment.

I am fully aware of the fact that the function of the reattached retina is almost as complex a problem as the function of the normal retina. It will therefore require considerable work on the part of physiologists, psychologists and ophthalmologists to define exactly how well a reattached retina functions after it has been detached for some time. Until the problem of the function of the reattached retina is solved scientifically—which will take considerable time because of the relative scarcity of adequate material—there is need for a plausible, workable idea in regard the probable functional outcome of an operation for retinal detachment before it can be recommended to the patient. It is true that in cases of partial but progressive detachment, even an anatomic cure or a reattachment without any return of function is a gain.
