What the evidence adds up to

Pulmonary blastoma is one of the least common primary malignant tumours of the lung. A 1974 case report described a tumour suspected at surgery that proved on histological examination to be a pulmonary blastoma. Three cases reported in 1988 described radiological, cytological, and histopathological features in detail. A 1990 journal article by Frederic B. Askin appeared in the American Journal of Clinical Pathology. A 1981 case of a four-year-old girl with pulmonary blastoma was diagnosed by two independent pathologists; the tumour’s histological demarcation was described as difficult and contested.

Because of the malignancy, a combined surgical-cytostatic-radiological therapy was decided upon, which the authors noted had not yet been done in that way. The patient was symptom-free for six months but then died from a brain metastasis. No other survival data, response rates, or sample sizes beyond these individual cases are reported in these abstracts. No drug is named in any of the abstracts.

What is still missing is any systematic trial, any patient stratification, any funding for prospective research, and any evidence that any drug has been tested against pulmonary blastoma in any number of patients.

AbstractPulmonary blastoma is one of the least common primary malignant tumors of the lung. There is considerable controversy as to its exact histogenesis; it is uncertain if it arises from a single cell or if it is a mixed tumor arising from two different cell lines. We report three patients with pulmonary blastomas, describing in detail their radiological, cytological, and histopathological features.
