What the evidence adds up to
An open study of levetiracetam as add-on therapy in nine patients with well-defined progressive myoclonic epilepsies and refractory myoclonus reported that five patients had improvement of their myoclonus score. The evaluation was semiquantitative, covering territory, intensity, and daily living activities. No controlled data exist for this drug in this population.
A 2021 Italian case series of 11 individuals with progressive myoclonic epilepsies (five with Unverricht–Lundborg disease, three with Lafora disease, two with sialidosis, one undetermined) reported that nine of the 11 improved their disability related to action myoclonus after treatment with perampanel; the two who did not improve both had Lafora disease. A systematic review of 104 patients from 11 case series found that more than half of patients receiving perampanel showed amelioration of action myoclonus and improved independence in daily life activities. Unverricht–Lundborg disease appeared to show the best clinical response. Almost all persons with active epilepsy achieved significant seizure reduction. Only 11% dropped out due to inefficacy. No prospective trials were found.
Botulinum neurotoxin was given to five patients with epilepsia partialis continua, a condition defined as continuous myoclonic jerking of a body part of cortical origin. In all five patients, botulinum neurotoxin treatment significantly reduced severity and frequency of myoclonus and led to pronounced improvement of speech and arm utilisation, regardless of the cause or duration of the epilepsy. The authors note that anti-epileptic therapy is frequently ineffective in this condition.
A 2024 case report describes a single patient with progressive myoclonic epilepsy type 7 due to a pathogenic variant in KCNC1 who experienced myoclonus improvement after epileptic seizures. The authors suggest this pattern may be a clue to the diagnosis of PME type 7, but no treatment data are provided.

Perampanel Improves Cortical Myoclonus and Disability in Progressive Myoclonic Epilepsies: A Case Series and a Systematic Review of the Literature
AbstractIntroduction: Progressive myoclonic epilepsies (PMEs) are a heterogenous group of genetic diseases presenting with epilepsy, cognitive impairment, and severe action myoclonus, which can severely affect daily life activities and independent walking ability. Perampanel is a recent commercially available antiseizure medication with high efficacy against generalized seizures. Some reports supported the role of perampanel in ameliorating action myoclonus in PMEs. Here, we aimed to describe a case series and provide a systematic literature review on perampanel effects on PMEs. Methods: We report the perampanel effectiveness on myoclonus, daily life activities, and seizures on an original Italian multicenter case series of 11 individuals with PMEs. Then, using the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines, we performed a systematic review on perampanel effect on myoclonus and disability in PMEs. We searched PubMed, Scopus, and Google Scholar articles on perampanel and PMEs up to June 2020. No prospective trials were found. We reviewed 11 case series manuscripts reporting 104 cases of different PMEs. Results: Here, we are reporting the effectiveness of perampanel in five individuals affected by Unverricht–Lundborg disease, three by Lafora disease, two by sialidosis, and one by an undetermined PME. Nine out of 11 individuals improved their disability related to the action myoclonus (two with Lafora disease did not). Among the 104 persons with PMEs collected by the systematic review, we found that more than half of the patients receiving perampanel exhibited an amelioration of action myoclonus and, consequently, of their independence in daily life activities. The Unverricht–Lundborg disease seemed to show the best clinical response to perampanel, in comparison with the other more severe PMEs. A significant seizure reduction was achieved by almost all persons with active epilepsy. Only 11% of PME patients dropped out due to inefficacy. Conclusions: Perampanel demonstrated a beneficial effect with regard to action myoclonus, disability, and seizures and was well-tolerated in people with PMEs, independently from their genetic diagnosis. Given the limited scientific evidence, broader prospective trials should be encouraged.

Sustained Effect of Botulinum Neurotoxin in Myoclonus Owing to Epilepsia Partialis Continua
AbstractBACKGROUND: Epilepsia partialis continua (EPC) is defined as continuous myoclonic jerking of a body part of cortical origin and often resembles a movement disorder. Unfortunately, anti-epileptic therapy is frequently ineffective. Currently, the effect of botulinum neurotoxin (BoNT) therapy in EPC is controversial. METHODS: We analyzed case histories, treatment protocols, and video documentation of 5 patients with EPC, who received BoNT therapy in our movement disorders unit. The Unified Myoclonus Rating Scale was used to document treatment effects. RESULTS: In all patients, BoNT treatment significantly reduced severity and frequency of myoclonus and led to pronounced improvement of speech and arm utilization, regardless of etiology or duration of EPC. CONCLUSION: BoNT represents a safe, effective treatment in EPC, offering long-term abatement of myoclonus. The substantial functional profit may hint at mechanisms beyond local impairment of neuromuscular transmission, such as modulation of maladaptive cortical plasticity, as observed in dystonia and poststroke spasticity.
