What the evidence adds up to
Primary ovarian insufficiency is defined as depletion or dysfunction of ovarian follicles with cessation of menses before age 40. There is no consensus on diagnostic criteria for adolescents, and delay in diagnosis is common. Once diagnosed, patients should be evaluated at least annually. Hormonal therapy aims to support bone, cardiovascular, and sexual health, not merely relieve symptoms. Estrogen therapy is recommended at least until the age of natural menopause unless a true contraindication exists. Referral to a reproductive endocrinology and infertility specialist should be offered when desired.
A 2022 systematic review examined six studies of treatments for primary ovarian insufficiency: one randomised controlled trial and five observational studies. Two studies tested intraovarian platelet-rich plasma infusion, two tested dehydroepiandrosterone supplements, one tested hormone replacement therapy, and one tested autologous adipose-derived stromal cells. The review found insufficient scientific evidence that any approach could help ovarian function recovery. Ovarian function markers showed inconsistent changes across studies. Among the patients in these studies, 26 (6.2%) reported spontaneous pregnancy. The review concluded that large randomised controlled trials are necessary and that data on ovarian function should be collected comprehensively.
Genetics plays a key role in the development of primary ovarian insufficiency. Better understanding of the genetics could lead to earlier diagnosis and broaden fertility options, but this remains an area for further research. A 2014 committee opinion notes that patients and families should be counselled on effects on future fertility, risk of comorbidities, and potential for genetic inheritance. Psychologic counselling should be offered because impaired self-esteem and emotional distress have been reported after diagnosis.
What is still missing: large randomised controlled trials with comprehensive collection of ovarian function markers, consistent evaluation methods across studies, and validated criteria for diagnosis in adolescents. No treatment has shown reliable evidence of restoring ovarian function.

Pharmacokinetic, pharmacodynamic, and clinical aspects of ovulation induction agents: A review of the literature
AbstractControlled ovarian hyperstimulation is a key step for successful outcomes of assisted reproductive technique cycle outcomes. Many medications are available, which are commonly useed solely or in combination to achieve multiple follicular development. Pharmacokinetic, pharmacodynamic, and clinical information of ovulation induction drugs deserve to be elucidated for every individual patient before commencing infertility treatment. New concepts and new treatment protocols are introduced as ovulation physiology is understood by infertility specialists. Increasing treatment success by minimizing aderse effects is a milestone of all ovarian stimulation protocols that use these novel interventions. Achievement of a satisfactory cycle outcome includes retrieval of sufficient oocytes, a single clinical pregnancy, and avoidance of ovarian hyperstimulation syndrome. In this review, we evaluate the current literature to determine the most reliable and relevant information about the most used ovulation induction drugs.

The Evaluation of Ovarian Function Recovery Following Treatment of Primary Ovarian Insufficiency: A Systematic Review
AbstractBackground: Primary ovarian insufficiency (POI) is gaining awareness as its prevalence increases and its effect on patients is extremely negative. To date, several therapies have been designed to treat POI, but the conclusions are conflicting, in part, due to inconsistent evaluation methods. Thus, we explore a multi-index of ovarian function assessment methods to evaluate the recovery of ovarian function after various therapies in order to evaluate effectiveness in a more comprehensive manner. Aim: The purpose of this review is to assess the effectiveness of various therapies to recover ovarian function in patients with POI. The primary outcome measures were anti-Müllerian hormone (AMH) levels, follicle stimulating hormone (FSH) levels, and antral follicle count (AFC). The secondary outcomes included the change of mean ovarian volume, menstruation recovery, and pregnancy rate. Methods: Our systematic searching including PubMed, Web of Science, Cochrane, and Embase databases was conducted to find all human clinical trial articles published from January 2000 to April 2021 and related to POI treatment, including the keywords: POI, AFC, and hormones. All prospective and retrospective studies exploring ovarian function recovery that include AFC, AMH levels, and FSH levels evolution throughout treatment were included. All patients included in the studies met the POI criteria described by the European Society for Human Reproductive Embryology (ESHRE) guideline. Results: Six studies were selected based on the criteria: one randomized controlled trial and five observational studies. Among them, two studies focused on the intraovarian platelet-rich plasma (PRP) infusion treatment, two studies focused on dehydroepiandrosterone (DHEA) supplements, one study focused on hormone replacement therapy (HRT), and one study focused on autologous adipose-derived stromal cells (ADSCs) treatment. There was insufficient scientific evidence that any approach could help ovarian function recovery in patients with POI because the ovarian function markers in each study had inconsistent changes with 26 patients (6.2%) reporting spontaneous pregnancy. Conclusion: Serum AMH levels, FSH levels, and AFC are sensitive indicators and reflect the evolution of ovarian function. Large randomized controlled trials are necessary, and the data on ovarian function should be collected comprehensively to evaluate the effectiveness of a variety of treatments.
AbstractThe diagnosis of primary ovarian insufficiency (POI) has untold effects on women and a better understanding alongside potential treatments are paramount to improve quality of life of these women. Various causes have been linked to the development of POI with genetics playing a key role. A better understanding of the genetics of POI could lead to earlier diagnosis and broaden fertility options. This chapter discusses previously known and more recently discovered genes that have been implicated in the development of POI. It explores the varying phenotypic expressions of some genes in different populations and areas for further research in the genetics of POI.

AbstractPrimary ovarian insufficiency (sometimes known as premature ovarian insufficiency) is a result of loss of ovarian follicular activity before the age of 40 years. It is an endocrine deficiency state in women, characterized by premature estrogen deprivation. In the absence of estrogen replacement, women experience bothersome menopause symptoms and a predisposition to accelerated aging and multimorbidity accumulation. Unless a true contraindication exists, estrogen therapy is recommended at least until the age of natural menopause. This Practice Pearl summarizes the clinical manifestations, diagnostic evaluation, and management of primary ovarian insufficiency.

Premature ovarian insufficiency: Genetic causes and treatment options. A literature review
AbstractPremature ovarian insufficiency is a syndrome characterized by hypergonadotropic ovarian insufficiency and the reduction of ovarian function before age 40. This leads to reproductive failures, metabolic changes, and a decrease in quality of life. Currently, occult and initial forms of premature ovarian insufficiency, which have their own diagnostic features and management tactics, can be figured out. The frequency of this syndrome is between 1.1 and 3.7% and the tendency for incidence to increase can be seen. This article is a literature review of the data available in the PubMed database (20052020), with international clinical guidelines taken into consideration. The genetic causes of premature ovarian insufficiency, clinical signs of this pathology and treatments options for such patients are included into the review. In addition, some features of assisted reproductive technology within this group are described.

AbstractLoss of normal ovarian function before the age of 40 characterizes primary ovarian insufficiency (POI), sometimes called premature ovarian failure or early menopause. Many women all around the globe deal with this serious reproductive health issue. The purpose of this study is to provide a broad perspective on how to identify and treat primary ovarian insufficiency. Symptoms, hormone profiles, and ovarian imaging help diagnose POI. Genetics, autoimmune illness, and medical intervention may cause it. After a proper diagnosis, affected women might get counselling and therapy. This review article covers primary ovarian insufficiency diagnosis and treatment. Affected women should be diagnosed early, counselled, and treated individually. Understanding POI and developing better drugs may improve these people's reproductive and general health.
