What the evidence adds up to
Pierson syndrome is caused by mutations in the laminin β2 (LAMB2) gene, which encodes the basement membrane protein laminin beta2. A review of 39 unrelated families found that most disease-causing LAMB2 mutations are truncating, consistent with loss of laminin beta2 function as the molecular basis. Missense mutations cluster in the N-terminal LN domain and are associated with a higher mean age at onset of renal disease and absence of neurologic abnormalities, suggesting some may be hypomorphic alleles. However, genotype alone does not explain the full range of clinical variability, and unidentified modifiers are likely to exist.
A two-month old boy in Pakistan presented with microcoria and congenital nephrotic syndrome, the characteristic features. Hypocalcaemia, hypoproteinaemia and a probable immunocompromised state from nephrotic syndrome led to seizures, hypothyroidism and urosepsis. Despite aggressive treatment with high dose antibiotics, ionotropic support, angiotensin-converting enzyme inhibitors, thyroxine replacement and nutritional support, the infant died from multiorgan disease including renal failure and septic shock.
A separate case report described a 3 year 10 month old girl with Pierson syndrome whose kidney biopsy showed features suggestive of Alport syndrome. Genetic testing revealed a LAMB2 mutation and a TRIM8 mutation. The patient was receiving supportive therapy.
What is still missing are disease-modifying treatments. No drug has been shown to alter the course of Pierson syndrome. The available evidence consists of case reports and a mutation review; no controlled trials exist. Patient stratification by mutation type may eventually matter, but the small number of patients and the rarity of the disease make trial design and funding difficult.

Mutations in the human laminin β2 (LAMB2) gene and the associated phenotypic spectruma
AbstractMutations of LAMB2 typically cause autosomal recessive Pierson syndrome, a disorder characterized by congenital nephrotic syndrome, ocular and neurologic abnormalities, but may occasionally be associated with milder or oligosymptomatic disease variants. LAMB2 encodes the basement membrane protein laminin beta2, which is incorporated in specific heterotrimeric laminin isoforms and has an expression pattern corresponding to the pattern of organ manifestations in Pierson syndrome. Herein we review all previously reported and several novel LAMB2 mutations in relation to the associated phenotype in patients from 39 unrelated families. The majority of disease-causing LAMB2 mutations are truncating, consistent with the hypothesis that loss of laminin beta2 function is the molecular basis of Pierson syndrome. Although truncating mutations are distributed across the entire gene, missense mutations are clearly clustered in the N-terminal LN domain, which is important for intermolecular interactions. There is an association of missense mutations and small in frame deletions with a higher mean age at onset of renal disease and with absence of neurologic abnormalities, thus suggesting that at least some of these may represent hypomorphic alleles. Nevertheless, genotype alone does not appear to explain the full range of clinical variability, and therefore hitherto unidentified modifiers are likely to exist.

Pierson syndrome presenting as Alport syndrome: A case report
AbstractWe report a 3 year 10 month old girl with Pierson syndrome. Kidney biopsy showed features suggestive of Alport syndrome. Genetic study report showed LAMB2 mutation which are considered to (Nephrotic syndrome, type 5 with or without ocular abnormalities and Pierson syndrome), TRIM8 mutation which are considered to (FSGS and neurodevelopmental syndrome).
