Endoscopic Management of Colonic Perforation Owing to Angiogenesis Inhibitors

AbstractAngiogenesis inhibitors have markedly improved, in combination with chemotherapy, the outcome of patients with metastatic solid tumors. Bevacizumab, the brand name of which is Avastin, is the first monoclonal antibody IgG1 used against the vascular endothelial growth factor involved in tumor proliferation. Gastrointestinal perforation is a well-known adverse event of bevacizumab therapy. Its surgical management is still controversial because of the high mortality rate of surgery. Here, we report a 48-year-old man with a history of metastatic renal cancer, who developed colonic perforation secondary to bevacizumab. This patient was treated successfully with endoscopic stenting.

Bevacizumab: a safe and effective treatment in a patient with advanced colorectal cancer and repeated removal of metastases

AbstractBevacizumab has been shown to be effective combined with chemotherapy for first-line treatment of advanced colorectal cancer, but little information is available about its efficacy and safety in patients who may be candidates for surgery at any time during the disease. The case history of a female patient with colorectal cancer, undergoing surgery for liver metastases and bilateral surgery for lung metastases at different time-points during her disease, is reported. Perioperative bevacizumab administration caused no complications either associated with surgery, in the early postoperative period, or in the subsequent months.
