What the evidence adds up to
Surgery remains the primary treatment for inverted papilloma. In a cohort of 37 cases, the recurrence rate was 8 per cent after endoscopic surgery and 21 per cent after combined endoscopic and external approaches. The extent of disease primarily determines the choice of surgical approach, with previous treatment, individual patient factors and surgical expertise as secondary determinants.
For recurrent respiratory papillomatosis, the current standard of care is surgical therapy aimed at complete removal or debulking of papillomas while preserving normal structures. Frequent recurrences and the need for repeated surgical interventions make this a frustrating experience. Many adjuvant therapies have been investigated, but no single treatment modality has proved effective in eradicating the disease. The molecular basis of HPV-related disease involves high expression of the viral oncoproteins E6 and E7, which inhibit apoptosis, accumulate DNA damage, and activate the cell cycle. More than 99.7 per cent of cervical cancer cases in women are linked to high-risk HPV, and in men high-risk HPV predominantly contributes to a subset of oropharyngeal cancers. What is still missing is a reliably effective adjuvant therapy or vaccine for recurrent respiratory papillomatosis, along with trial designs that account for the heterogeneity of disease extent and patient factors, and adequate funding for pharmaceutical research in this rare condition.

AbstractSurgery offers the optimum modality of treatment for inverted papilloma although a considerable range of operative approaches have been described. The results are presented in a cohort of 37 cases treated by both endoscopic and combined endoscopic and external approaches with a recurrence rate of eight and 21 per cent respectively. This series is compared with those in the literature and demonstrates that it is extent of disease which primarily determines the choice of surgical approach, with previous treatment, individual patient factors and surgical expertise as secondary determinants.

Novel treatments for recurrent respiratory papillomatosis
AbstractRecurrent respiratory papillomatosis is a rare but often severe disease. Although benign in histology, epithelial proliferations may result in progressive hoarseness, stridor, obstruction of the airway and respiratory distress. The current standard of care is surgical therapy with a goal of complete removal or debulking of papillomas and preservation of normal structures. Frequent recurrences and the need for repeated surgical interventions make this treatment a frustrating experience for both the patient and the physician. Many adjuvant therapies have been investigated but no single treatment modality proved to be effective in eradicating recurrent respiratory papillomatosis. In contrast to HIV, cytomegalovirus and hepatitis B pharmaceutical research has been less successful with human papilloma virus vaccines for a variety of reasons. This review focuses on the current status of recurrent respiratory papillomatosis and on future directions of prevention and therapy.

AbstractIntroduction: Squamous cell papilloma is a benign tumor whose pathogenesis is generally related to the human papillomavirus. Despite affecting several organs, we did not find cases reported in the nails. Case Presentation: A 67-year-old female presented with a painful lesion in the nail of the right hallux that started in 2021, with an erythematous appearance evolving to black and expansive growth. Due to the evolution and symptoms, she opted for excision, with a histopathological report of squamous cell papilloma. Complementary treatment with imiquimod was performed, with good healing and pain control. Discussion: Due to the lack of previous reports, treatment was based on therapeutic options for papillomas located in other regions. Excision was performed, followed by application of imiquimod with good response. The prognosis is favorable; however, further studies are needed to elucidate the diagnosis and management of nail squamous cell papilloma.
