What the evidence adds up to
Papillary carcinoma can arise in the thyroid, breast, larynx, and sinuses. A 1996 case report describes a papillary thyroid carcinoma that metastasised to the sphenoid-ethmoid sinuses and skull base; the tumour contained areas of insular and anaplastic degeneration with strong positive staining for p53 oncogene, and management included surgery, radioactive iodine, and external beam radiation. A 2018 case from Brazil reports laryngeal papillary carcinoma as a rare entity, though on histopathological study the lesion was classified as papillary squamous cell carcinoma.
A 2022 study of 42 patients with 45 papillary breast carcinoma lesions found that mammography, ultrasound, and magnetic resonance imaging features overlap between subtypes, and that ultrasound and MRI are complementary for revealing specific morphological characteristics. No survival or response rates are reported in that imaging study.
A 2017 review of 49 reports involving 53,606 patients with papillary thyroid tumours concludes that the past emphasis on cytologic features to define papillary tumours has not produced ideal pairwise agreement between pathologists and has generated incidence and survival data suggesting overdetection and overdiagnosis. Updated survival analyses demonstrate excellent long-term survival for most patients diagnosed with papillary carcinomas, and the review notes that a reemphasis on diagnostic tissue patterns should improve agreement and reduce incidence.
No controlled trial data exist for any drug in these abstracts. What is missing is prospective trial design, patient stratification by tumour subtype and molecular markers, and funding for studies that test repurposed agents against papillary carcinomas at any site.

Digital Mammography, Ultrasound and Magnetic Resonance Imaging Characteristics in Differential Diagnosis of Papillary Carcinoma Subtypes of the Breast and Diagnostic Challenges
AbstractObjective: We aimed to investigate mammography (MG), ultrasound (US), and magnetic resonance imaging (MRI) findings of papillary breast carcinoma subtypes and to compare the diagnostic features and performance of the imaging method in distinguishing subtypes. Materials and Methods: Forty-two patients presenting with pathological diagnosis of 45 papillary carcinoma lesions, between 2014 and 2019, were included. Cases were assigned to five subgroups according to the latest World Health Organization (WHO) classification. The clinical characteristics (n = 45) and imaging features of each pathological subgroup were retrospectively related to imaging findings from US (n = 45), MG (n = 37), and breast MRI (n = 23), and further compared. Results: = 0.017). Conclusion: Radiological findings of papillary carcinomas overlap with each other. US and MRI are complementary when revealing specific morphological characteristics.

Defining Papillary Carcinoma of the Thyroid
AbstractOBJECTIVES: To review how changes in the pathologic definitions for papillary tumors of the thyroid during recent decades have affected outcomes for patients with these tumors. METHODS: Forty-nine previous reports or studies involving collectively 53,606 patients were reviewed, and new analyses were performed on the data to include analyses of agreement, incidence, survival, and diagnostic categories. RESULTS: The past emphasis on cytologic features to define papillary tumors has not resulted in ideal pairwise agreement between pathologists and has produced incidence and survival data suggesting overdetection and overdiagnosis. Most recently, tissue patterns have been reemphasized. CONCLUSIONS: With the recent reemphasis on diagnostic tissue patterns (over cytologic criteria), agreements between pathologists for the diagnosis of papillary tumors should improve, and the incidence of papillary carcinoma should decline. Nevertheless, updated survival analyses demonstrate excellent long-term survival for most of those diagnosed with papillary carcinomas.
