What the evidence adds up to

Ovarian small cell carcinoma is a rare, highly aggressive tumour with two recognised types: the hypercalcaemic type (SCCOHT) and the pulmonary type (SCCOPT). A 2022 case report describes a 23-year-old woman who presented with acute abdomen due to adnexal torsion. After unilateral salpingo-oophorectomy, histopathological evaluation by a team of pathologists confirmed hypercalcaemic small cell carcinoma. The patient refused reoperation, relapsed during chemotherapy, and died six months after initial diagnosis. The authors state that comprehensive information on this cancer is lacking, cytopathological diagnosis remains challenging, and treatments are not usually effective.

A 2013 case series of three patients with small cell carcinoma of the ovary reports rapid progression and spread of disease despite multifactorial therapies. The authors note that no standard treatment has been established for either treatment modalities or outcomes, and that different treatment options administered to these patients were unsuccessful in achieving satisfactory outcomes or disease control. The prognosis is described as very poor with an extremely high mortality rate, except for very rare cases reported in the literature.

A 2015 review of ovarian carcinomas in general describes the heterogeneity of epithelial ovarian cancers and states that immunohistochemistry and molecular testing help pathologists decipher this heterogeneity. It presents improved molecular understanding as a first step toward developing targeted therapies, but does not report any specific treatment results for small cell carcinoma of the ovary.

What is still missing are effective treatments validated in clinical trials, reliable diagnostic criteria that can be applied consistently by pathologists, and any understanding of which patient subgroups might respond to existing therapies. No drug has been shown to improve outcomes in this disease in any of the reported cases.

Pathobiology of ovarian carcinomas

AbstractOvarian tumors comprise a heterogeneous group of lesions, displaying distinct tumor pathology and oncogenic potentiel. These tumors are subdivided into three main categories: epithelial, germ cell, and sex-cord stromal tumors. We report herein the newly described molecular abnormalities in epithelial ovarian cancers (carcinomas). Immunohistochemistry and molecular testing help pathologists to decipher the significant heterogeneity of this disease. Our better understanding of the molecular basis of ovarian carcinomas represents the first step in the development of targeted therapies in the near future.

Ovarian small cell carcinoma by ovarian torsion feature

AbstractINTRODUCTION AND IMPORTANCE: Ovarian small cell carcinomas are a rare type of ovarian cancer that is highly aggressive and consists of two distinct types the hypercalcemic type (SCCOHT) and pulmonary type (SCCOPT). CASE PRESENTATION: A 23 years old girl was admitted to the emergency room with the presentation of acute abdomen. The ultrasound and Magnetic resonance imaging revealed a right adnexal huge mass with adnexal torsion. In laparotomy, she underwent unilateral salpingo-oophorectomy due to ovarian torsion and possible malignancy. The histopathological evaluation was challenging and was finalized by a team of pathologists as hypercalcemic small cell carcinoma. She refused reoperation and unfortunately relapsed during chemotherapy and died 6 months after the initial diagnosis. CLINICAL DISCUSSION: Conclusion: We do not yet have comprehensive information on small cell ovarian cancer. Cytopathology diagnosis is still challenging and the treatments are not usually effective. Further clinical trials and studies are recommended to find appropriate treatments for these patients.
