What the evidence adds up to

One hundred and eleven women with FIGO Stages III and IV ovarian adenocarcinoma were treated with melphalan as first chemotherapy. The overall response rate was 19.8%, with 10.8% achieving a complete response. Women who had not received prior radiation therapy had a statistically significant better response. Median survival was statistically significantly improved only in those who achieved a complete response compared to those with a partial response, no change, or progression. The authors concluded that maximal surgery and combination chemotherapy were needed to improve on these results.

Two review articles from 2017 note that epithelial ovarian cancers are the most common ovarian malignancies and are usually advanced when diagnosed in more than two thirds of patients because they remain asymptomatic until metastasis. These cancers have the highest mortality rate among female reproductive system malignancies. The reviews state that the incidence of different kinds of ovarian cancer has not received due attention and that they remain challenging issues.

No drug other than melphalan is mentioned in these abstracts. The 1978 study is small by modern standards and the response rate is low. The 2017 reviews provide no new treatment data. What is still missing is any evidence from a properly powered, randomised trial that tests a repurposed drug specifically against ovarian adenocarcinoma, with clear patient stratification by prior treatment and tumour genetics, and with funding adequate to follow survival endpoints rather than just response rates.

Melphalan Chemotherapy in Advanced Ovarian Carcinoma

AbstractOne hundred and eleven consecutive women with FIGO Stages III and IV ovarian adenocarcinoma were treated with melphalan as their initial chemotherapy. The response rate was 19.8% with 10.8% having a complete response. There was a statistically significant improvement in response in those women with no prior radiation therapy. Moreover, there was a statistically significant improvement in median survival in those patients achieving a complete response to melphalan as compared to those with a partial response, with no change or progression of their malignancy. A review of the results of maximal surgery and combination chemotherapy is presented as a plan of therapy to improve upon the results of melphalan chemotherapy in women with advanced ovarian carcinoma.

In this review, the papers published until early January 2017 that were conducted to study the Ovarian Adenocarcinoma were selected. Results: The ovary is in the form of two small glands on both sides of uterus that is covered by the end of ovarian tube. Ovarian epithelial cancers are the most common cancers that remain asymptomatic until metastasis; they have the highest rate of mortality among all kinds of malignancies of female reproductive system. Discussion and conclusion: The incidence of different kinds of ovarian cancer are included among the cases that have not been given due attention so far.
