What the evidence adds up to
In 1955, four elderly patients with ophthalmic herpes zoster were treated with ACTH. The authors reported successful relief of acute pain and prevention of serious complications, but the report is a case series of four patients with no control group, no randomisation, and no quantitative survival or response rates given. The paper itself notes that herpes zoster is usually benign and self-limiting, and that innumerable remedies had previously failed to relieve pain or prevent sequelae.
A 2018 review states that herpes zoster has a lifetime risk of 33%, that herpes zoster ophthalmicus occurs in 10–20% of all zoster cases, and that ocular involvement occurs in 50% of those with ophthalmic zoster. No treatment efficacy data are provided in that abstract.

AbstractAcyclovir and steroids in herpes zoster keratouveitis SIR, We read McGill and Chapman's article' with much interest. While we agree with them regarding steroid dependency of many of the inflammatory ocular complica- tions of zoster with the attendant difficulty of withdrawal, and that there are significant complications of long term topical steroids, we would not agree to withholding these valuable drugs. In the Zoster Clinic carefully controlled topical steroids have been used successfully in the inflam- matory complications of ophthalmic zoster for over 10 years.
