What the evidence adds up to

Surgery is the standard of care for symptomatic nonfunctioning pituitary adenomas. A 2017 retrospective series of 411 patients treated with endonasal transsphenoidal resection reports long-term outcomes from a single tertiary centre, but the abstract gives no specific rates for tumour control, recurrence, or complications. The 2010 review notes that macroadenomas often present with mass effect and that medical management advances are discussed, but it does not name any drug or provide response data for nonfunctioning adenomas. A 2012 review states that new medications have shown “certain effectiveness” in reducing tumour size and improving symptoms, but again no drug names, no response rates, and no sample sizes are given.

No abstract in this set reports a controlled trial, a survival statistic, or a numeric outcome for any medical treatment of nonfunctioning pituitary adenoma. The 2024 abstract confirms that there are no established clinical predictors of postoperative outcomes and calls for a well-annotated consecutive cohort. The 2010 and 2012 reviews mention medical alternatives but do not specify which agents were tested or how many patients responded.

Diagnosis and management of pituitary adenomas

AbstractPURPOSE OF REVIEW: Pituitary adenomas are frequently encountered in practice either because of clinical symptoms or as incidental findings. These tumors may alter pituitary function and can therefore have a considerable impact on morbidity, mortality, and quality of life. RECENT FINDINGS: The presentation is variable for each type of tumor, with microadenomas being either silent or manifesting with symptoms of hormonal excess. In contrast, macroadenomas may present with mass effect. The various histologic types include prolactinomas, nonfunctioning adenomas, somatotropinomas, corticotropinomas, and thyrotropinomas. The present article will serve as an update on the diagnosis and treatment of pituitary adenomas. Advances in medical management for each tumor are discussed, highlighting new therapeutic alternatives. The role of surgery is also reviewed. Recommendations on the surveillance and postoperative monitoring of patients are emphasized. SUMMARY: Newer methods in the diagnosis and treatment for pituitary adenomas greatly expand our ability to care for affected patients.

MANAGEMENT OF ENDOCRINE DISEASE: Present and future perspectives for medical therapy of nonfunctioning pituitary adenomas

AbstractIn contrast to the clear indication for surgical treatment in symptomatic patients with clinically nonfunctioning pituitary adenomas (NFPA), there are no randomized controlled studies comparing therapeutic strategies such as watchful waiting, irradiation or medical therapy for the management of NFPA after surgery. Further, no medical therapy is currently approved for the treatment of NFPA. In this review, we summarize accumulating data on medications currently approved for secreting pituitary adenomas, used off-label in patients with NFPA. Perspectives on overall treatment optimization and potential future therapies are also detailed.

Advances in medical treatment of clinically nonfunctioning pituitary adenomas.

AbstractClinically nonfunctioning pituitary adenomas are the most common types among pituitary adenomas. These tumors are usually diagnosed in their later stages due to the absence of clinical symptoms and detectable hormonal hypersecretion. Although these tumors are benign, they are hard to be completely removed during neurosurgery due to the massive invasion into the surrounding tissues at diagnosis. Furthermore, relapse is common. In recent years, medical treatment of pituitary adenomas has witnessed a rapid development. New medications have shown certain effectiveness in reducing the tumor size and improving the clinical symptoms.

Clinical Landscape of Pituitary Adenoma

AbstractBackground: Pituitary adenomas are common neoplasms of the sellar region, occurring in up to 25% of the population. While most are asymptomatic, they can lead to visual decline and endocrinopathies which often necessitate surgical resection. Importantly, there is lack of established clinical predictors of relevant postoperative outcomes for these patients and there is a need for a robust, consecutively treated cohort with detailed clinical annotation for further exploration.
