What the evidence adds up to

The 2009 paper on symmetric lipomatoses in female patients describes six women: three with benign (multiple) symmetric lipomatosis and three with female zonal obesity. In the benign symmetric lipomatosis cases, fat deposition was massive, firm, and symmetric around the neck and shoulder girdle, and was clearly associated with alcohol abuse or liver disease. No malignant tumours of the upper airways were found. In the female zonal obesity cases, fat accumulated mainly at the buttocks and thighs but spared the feet and hands; tenderness was common, and there was familial predisposition. Histology in both groups showed normal fatty tissue that was neither encapsulated nor septally divided. The authors propose that the term symmetric lipomatoses covers two separate disorders.

A 1989 case report describes a 61-year-old man with multiple symmetric lipomatosis who had the striking appearance characteristic of the condition and a history of alcohol abuse. He had undergone surgical treatment three times at other hospitals before admission. A 2009 case report describes a Japanese man with benign symmetric lipomatosis associated with atopic dermatitis and chronic alcohol abuse. The paper notes that histologically, cells in the abnormal lipomatous areas are indistinguishable from normal fat.

No drug treatment is mentioned in any of these abstracts. The only intervention described is surgery, and in the 1989 case it was repeated without preventing recurrence. There is no evidence from these reports of any pharmacological therapy, no response rates, no survival data, and no controlled trials. What is still missing is any clinical trial of a drug for this condition, any systematic attempt to stratify patients by aetiology (alcohol-related versus familial versus other), and funding for such research.

Symmetric Lipomatoses in Female Patients

AbstractBACKGROUND: Symmetric lipomatoses are characterized by marked symmetric deposition of diffusely distributed fatty tissue. Though relatively common disorders, they are rather rarely reported in the literature, possibly being misdiagnosed as general obesity. While the differential diagnosis of symmetric lipomatosis versus general obesity may not appear difficult in males, it is obviously problematic in females. OBSERVATIONS: We describe the findings in 6 representative female patients with symmetric lipomatoses: 3 with benign (multiple) symmetric lipomatosis and 3 with female zonal obesity. The former disorder was characterized by massive, firm, symmetric fat deposition predominantly around the neck and shoulder girdle and was clearly associated with alcohol abuse and/or liver disease. There were no malignant tumors of the upper airways. In the latter case, fatty tissue had accumulated mainly at the buttocks and thighs but characteristically spared the feet and hands. Tenderness was a common symptom. This disorder showed familial predisposition. Histology in both cases revealed normal fatty tissue which was neither encapsulated nor septally divided. CONCLUSIONS: We suggest that the term 'symmetric lipomatoses' refers to two separate disorders, benign (multiple) symmetric lipomatosis and female zonal obesity.

AbstractA case of multiple symmetric lipomatosis in a 61-year-old man is described. The patient had the striking appearance that characterizes multiple symmetric lipomatosis. He had a history of alcohol abuse. Before admission to our institution, he underwent surgical treatment three times at other hospitals. We describe his clinical course and review the literature.

It was subsequently characterized by Madelung in 1888 and Launois & Bensaude in 1898 (for review see ref 1). Histologically, cells in the abnormal lipomatous areas are indistinguishable from those in normal fat (2).
