What the evidence adds up to

Monocytic leukemia is described in a 1947 case report as a disease with an erratic course, where momentary observations can lead to erroneous conclusions. The report emphasises studying the natural history of the disease to reconcile conflicting opinions about its origin and characteristics. A 1967 clinical appraisal of chronic monocytic leukemia notes that some authors question its existence as a distinct entity. Among 25 patients at Johns Hopkins Hospital since 1955, the syndrome was mainly characterised by a long preleukemic phase with proliferative dysfunction of various blood elements, often presenting as refractory anemia up to 16 years before monocytes were numerous enough for diagnosis.

A 1978 report describes two adolescents whose early detection and treatment of acute monocytic leukemia was hampered by positive slide tests for infectious mononucleosis. The authors note this is important given the development of specific therapy effective for certain subtypes of nonlymphocytic leukemia at that time. A 1988 study using a human acute monocytic leukemia cell line (THP-1) found that its culture medium could induce normal human monocytes to spread, an ability not found in culture medium from a promyelocytic leukemia cell line (HL-60) or a diploid human fibroblast cell line. The monocyte spreading activity was found in a fraction with a molecular weight of about 70,000, which also increased production of angiotensin converting enzyme and Fc receptor sites for IgG on normal human monocytes.

No clinical trial data for any drug treatment of monocytic leukemia is provided in these abstracts. The 1947 paper explicitly warns against drawing conclusions from inconstant observations. The 1967 paper describes a syndrome whose very existence is questioned. What is missing is any controlled trial of a specific therapy, any modern molecular stratification of patients, and any funding for prospective studies that could move beyond case reports and cell-line experiments.

MONOCYTIC LEUKEMIA A CASE REPORT ILLUSTRATING VARIATIONS IN THE CLINICAL PICTURE

AbstractAbstract Monocytic leukemia is an entity that has engendered a variety of divergent clinical and histologic opinions. A case is presented in which prolonged and intensive clinical and laboratory observations demonstrate the erratic course of this disease and illustrate the erroneous conclusions that may be derived from inconstant, momentary observations of this dynamic process. Study of the "natural history" of monocytic leukemia yields observations which tend to reconcile many of the hitherto conflicting opinions regarding this disease. The origin and characteristics of the monocytic cell are discussed, and its probable derivation from mesenchyma is emphasized.

MONOCYTIC CHLOROMA

AbstractThe citation of rare cases is of value only so far as it makes for more exact diagnosis, which in turn tends to make the treatment of disease less empirical. The purpose of this paper is to report a case in which there were a blood picture of monocytic leukemia and pathologic involvement of the reticulo-endothelial system. Monocyte Activating Factor Originally Found in Sarcoidosis Sera

AbstractAbstract The culture medium of a human acute monocytic leukemia cell line (THP‐1) was able to induce normal human monocytes to spread. No such ability was found either in the culture medium of a promyelocytic leukemia cell line (HL‐60) nor in that of a diploid human fibroblast cell line (Flow 7000). Gel filtration of the culture medium of THP‐1 cells on a size exclusion column (TSKgel G3000SW) revealed that the most obvious monocyte spreading activity was found in the fraction eluted at the position with a molecular weight of about 70,000. This fraction was also able to increase production of angiotensin converting enzyme and Fc receptor sites for IgG on normal human monocytes.
