What the evidence adds up to
Germ cell tumours are one of the few cancers for which highly effective treatment has been found over the past quarter-century, and a high proportion of patients are entered into large cooperative clinical trials. The tumours are diverse and management complex; referral to specialist oncology centres is considered necessary in all cases. A 2014 study from Saint Petersburg reports that a lack of awareness among young men and general practitioners often causes late medical aid appealability and potential diagnostic errors, and that reducing the time between a patient’s first contact and final diagnosis may contribute to earlier detection, reducing the amount of treatment and improving survival rates.
A 2015 case report describes a 14-year-old girl with a suprasellar mixed malignant germ cell tumour treated with complete surgical resection, chemotherapy, and radiation therapy. The authors state that, based on a literature review, platinum-based chemotherapy regimens combined with radiation therapy provide the best outcome in terms of survival rates, though they note this is controversial and debated. Twenty months post-treatment the patient was doing well and neurologically intact, but the authors caution that only time will tell the efficacy of that treatment plan.

What is still missing are large, prospective trials that stratify patients by tumour subtype and location, standardised protocols for when to use surgery versus biopsy alone, and long-term follow-up data beyond single case reports. Funding for multicentre registries and randomised comparisons of platinum-based regimens with or without radiation would clarify which combinations truly improve survival without excessive toxicity.

AbstractThe most remarkable advance in therapeutic oncology during the past 10-15 years has been in the treatment of germ cell tumours. A high proportion of patients with these cancers are now entered into large cooperative clinical trials, which hold the key to improvements in management in the future. These tumours are diverse and their, management complex; referral to specialist oncology centres with the wide range of expertise necessary to treat these fascinating neoplasms should be considered in all cases.

Final Report of the First Refractory Germ Cell Tumor Treated with Sunitinib Malate
AbstractPatients with advanced germ cell tumors can be cured with cisplatin-based chemotherapy, but the outcome remains unsatisfactory for patients with relapsed disease, including those patients with refractory disease after bone marrow transplantation. Targeted therapies have changed the standard of care for many advanced solid tumors. We have identified, in the literature, potential targets for the treatment of refractory germ cell tumors, and applied to a patient with a refractory disease. We chose sunitinib for this purpose. To our knowledge, this is the first case to be treated with sunitinib, and we have found a promising activity.

Early lesions are frequently asymptomatic, but advanced tumors can cause a number of symptoms, such as vague abdominal pain and distention. Acute symptoms could be due to the rupture or twisting of the tumor. The prognosis is often favorable, and chemotherapy is frequently administered after conservative surgery, which involves complete excision of the tumor and preservation of reproductive function. We describe a case of MGCT in an 8-year-old girl and highlight the diagnostic conundrum as well as the optimal management strategy.

Suprasellar mixed malignant germ cell tumor: Case report and review of the literature
AbstractA 14-year-old girl with a suprasellar mixed malignant germ cell tumor was treated with complete surgical resection, chemotherapy, and radiation therapy. We performed a literature search to explore the current treatment options and prognosis of such a diagnosis. In reviewing the literature on mixed germ cell tumors, it appears that platinum-based chemotherapy regimens combined with radiation therapy provide the best outcome in terms of survival rates. Although controversial and debated, initial therapy should include surgical resection or a stereotactic biopsy when resection is not possible. By obtaining tissue for histopathology, one can tailor treatment plans accordingly and provide greater prognostic accuracy. In this case report, all three therapeutic modalities were used in the treatment of a mixed malignant germ cell tumor. Thus far, 20 months posttreatment, the patient is doing well and is neurologically intact, but diagnosis and treatment were relatively recent. Only time will tell the efficacy of this treatment plan.
