What the evidence adds up to

A 2022 study of congenital mirror movements (CMM) identified a novel variant in the DCC gene located in a conserved motif of the cytoplasmic tail that is predicted to bind the WAVE regulatory complex (WRC). Biochemical and axon guidance assays showed that this CMM-associated DCC variant is pathogenic because it disrupts the interaction between DCC and the WRC. The DCC-WRC interaction is evolutionarily conserved and required for Netrin-1 mediated commissural axon outgrowth and guidance. The study concluded that the WRC is a pivotal component of Netrin-1/DCC signalling and that the finding provides a molecular mechanism explaining how genetic variants in DCC may lead to CMM.

A 2025 systematic review of mirror therapy for musculoskeletal injuries of the hand and wrist included seven studies with samples ranging from 22 to 40 participants, totalling 220 participants. Five studies combined mirror therapy with active exercise and had comparator groups performing active exercises for the same duration. Mirror therapy interventions lasted 20 to 75 minutes per session, two to five times a week for three to eight weeks. Mirror therapy reduced pain, improved range of motion, strength, and function in most studies. Greater improvements for the mirror therapy groups than the comparator group were reported for pain, range of motion, and function in 20% to 75% of the studies. Risk assessment resulted in one high, four moderate, and two low quality studies. The authors concluded that moderate quality evidence suggests mirror therapy may be effective for these outcomes, but that further investigation with larger trials and more homogenous interventions is warranted.

A 2018 study on mirror therapy protocols for the hemiparetic upper extremity suggested that gross motor activities and movements yield the most realistic mirror therapy protocols. A 2019 review described mirror therapy as a treatment that uses flat-panel imaging combined with rehabilitation training, allowing patients to observe motion of unaffected limbs and visualise motion of the affected side through optical illusion, visual feedback, and virtual reality. That review noted mirror therapy is widely used in rehabilitation for dysfunction, sensation, dyskinesia, and language, especially for pain and exercise capacity, and reported possible mechanisms.

What is still missing for mirror movements is a direct test of any drug in patients with CMM. The 2022 study identifies a molecular target in DCC signalling, but no compound has been shown to restore the disrupted DCC-WRC interaction in humans. For mirror therapy as a rehabilitation approach, the evidence base remains limited by small sample sizes, heterogeneous protocols, and moderate study quality. Larger trials with standardised interventions and longer follow-up are needed, and no drug has been proposed or tested for the condition.

A human DCC variant causing mirror movement disorder reveals an essential role for the Wave regulatory complex in Netrin/DCC signaling

AbstractSUMMARY The axon guidance cue, Netrin-1, signals through its receptor DCC to attract commissural axons to the midline. Pathogenic variants in DCC frequently lead to congenital mirror movements (CMM), but how these variants impact DCC function is largely unknown. Screening of DCC in individuals with CMM recently revealed a novel variant located in a conserved motif in the cytoplasmic tail of DCC that is predicted to bind to a central actin nucleation promoting factor, the WAVE regulatory complex (WRC). Here, we use biochemical and axon guidance assays to show that this CMM-associated DCC variant is pathogenic by disrupting the interaction between DCC and the WRC. This DCC-WRC interaction is evolutionarily conserved and is required for Netrin-1 mediated commissural axon outgrowth and guidance. Together, we identify the WRC as a pivotal component of Netrin-1/DCC signaling and further provide a molecular mechanism explaining how genetic variants in DCC may lead to CMM.

Learning to Cope with Mirror Movements in Activities of Daily Living: Effects of Targeted Bimanual Therapy in Children with Unilateral Spastic CP

AbstractBackground and Purpose: Many children with USCP show involuntary movements of the other hand during voluntary unimanual movements, so-called mirror movements (MM). These MM negatively influence many bimanual activities of daily living. But no therapeutic regime is described so far which specifically addresses these MM. Our aim was to evaluate if a targeted bimanual intensive therapy reduces the negative impact of MM on bimanual activities, and reduces the severity of MM in these children in general.

Effectiveness of mirror therapy to treat musculoskeletal injuries of the hand and wrist: A systematic review

AbstractBACKGROUND: Mirror therapy is an effective intervention for improving outcomes when an affected extremity has severe movement restrictions or pain. PURPOSE: To investigate the effects of mirror therapy interventions on musculoskeletal injuries of the hand/wrist. STUDY DESIGN: Systematic review. METHODS: This systematic review was conducted in accordance with Cochrane and PRISMA guidelines and registered with Open Science Framework (DOI:XXX). The search was conducted in EMBASE, PubMed, and Scopus in April 2024. Search terms included hand and wrist injuries, mirror therapy, pain, range of motion, strength, and function. Descriptive synthesis was used to summarize the data for interventions and outcomes. The Cochrane RoB 2 tool was used for quality assessment. RESULTS: Samples ranged from 22 to 40 participants with a total of 220 participants. Five studies combined mirror therapy with active exercise and had comparator groups performing active exercises for the same duration. Mirror therapy intervention ranged from 20 to 75 minutes per session, two to five times a week for 3-8weeks. Mirror therapy interventions reduced pain, improved range of motion, strength, and function in most studies. Greater improvements for the mirror therapy groups than the comparator group were reported for pain, range of motion, and function in 20%-75% of the studies. Risk assessment for the seven studies resulted in one high, four moderate, and two low quality studies. CONCLUSIONS: Moderate quality evidence suggests that mirror therapy interventions may be effective at reducing pain, improving range of motion, strength and function for musculoskeletal hand and wrist injuries. Further investigation is warranted with larger trials with more homogenous interventions.
