What the evidence adds up to
Mirror therapy was tested in 15 amputee patients with phantom limb pain in a quasi-experimental study from 2016. Forty minutes of training was given, and patients practised at home for four weeks. The therapy produced a significant decrease in pain severity measured on a 0–10 scale. Patients not using a prosthesis had greater benefit. The authors concluded mirror therapy is safe, economical, and easy to use, but the sample was small and there was no control group.
A 2022 follow-up study of a previous randomised controlled trial examined long-term effects of movement-based and task-based mirror therapy in people with poststroke hemiparesis. The authors reported significant long-term improvements in occupational performance and some significant benefits in the task-based group. However, they stated that study limitations inhibit drawing meaningful applications to the field of occupational therapy.

The risk of bias assessment rated one study high, four moderate, and two low quality. The authors concluded that moderate quality evidence suggests mirror therapy may be effective, but larger trials with more homogenous interventions are needed.
A 2020 mouse study examined the role of floor plate Netrin-1 in corticospinal tract development. Depletion of floor plate Netrin-1 in conditional knock-out mice critically disrupted midline crossing of the corticospinal tract, leading to abnormal ipsilateral projections and symmetric movements. The authors described this as a unique mouse model recapitulating characteristics of human congenital mirror movements through abnormal corticospinal tract decussation. No human drug intervention was tested.

The Effect of Mirror Therapy on the Management of Phantom Limb Pain
AbstractOBJECTIVES: In the last two decades, mirror therapy has become a frequently used method of managing phantom limb pain (PLP). However, the role of nurses in mirror therapy has not yet been well defined. This study examined the effect of mirror therapy on the management of PLP, and discusses the importance of mirror therapy in the nursing care of amputee patients. METHODS: This quasi-experimental study was conducted in the pain management department of a university hospital and a prosthesis clinic in İstanbul, Turkey, with 15 amputee patients who had PLP. Forty minutes of practical mirror therapy training was given to the patients and they were asked to practice at home for 4 weeks. Patients were asked to record the severity of their PLP before and after the therapy each day using 0-10 Numeric Pain Intensity Scale. RESULTS: Mirror therapy practiced for 4 weeks provided a significant decrease in severity of PLP. There was no significant relationship between the effect of mirror therapy and demographic, amputation or PLP-related characteristics. Patients who were not using prosthesis had greater benefit from mirror therapy. CONCLUSION: Mirror therapy can be used as an adjunct to medical and surgical treatment of PLP. It is a method that patients can practice independently, enhancing self-control over phantom pain. As mirror therapy is a safe, economical, and easy-to-use treatment method, it should be considered in the nursing care plan for patients with PLP.

Loss of floor plate Netrin-1 impairs midline crossing of corticospinal axons and leads to mirror movements
AbstractAbstract In human, execution of unimanual movements requires lateralized activation of the primary motor cortex, which then transmits the motor command to the contralateral hand through the crossed corticospinal tract (CST). Mutations in NETRIN-1 alter motor control lateralization, leading to congenital mirror movements. To address the role of midline Netrin-1 on CST development and subsequent motor control, we analyzed the morphological and functional consequences of floor-plate Netrin-1 depletion in conditional knock-out mice ( Shh::cre;Ntn1 lox/lox mice). Here, we show that depletion of floor plate Netrin-1 critically disrupts midline crossing of the CST, whereas the other commissural systems are mostly preserved. The CST defect results in abnormal but functional ipsilateral projections, and is associated with abnormal symmetric movements. Therefore, our study reveals a new role for Netrin-1 in CST development. It also describes a unique mouse model recapitulating characteristics of human congenital mirror movements, through abnormal CST decussation.

Effectiveness of mirror therapy to treat musculoskeletal injuries of the hand and wrist: A systematic review
AbstractBACKGROUND: Mirror therapy is an effective intervention for improving outcomes when an affected extremity has severe movement restrictions or pain. PURPOSE: To investigate the effects of mirror therapy interventions on musculoskeletal injuries of the hand/wrist. STUDY DESIGN: Systematic review. METHODS: This systematic review was conducted in accordance with Cochrane and PRISMA guidelines and registered with Open Science Framework (DOI:XXX). The search was conducted in EMBASE, PubMed, and Scopus in April 2024. Search terms included hand and wrist injuries, mirror therapy, pain, range of motion, strength, and function. Descriptive synthesis was used to summarize the data for interventions and outcomes. The Cochrane RoB 2 tool was used for quality assessment. RESULTS: Samples ranged from 22 to 40 participants with a total of 220 participants. Five studies combined mirror therapy with active exercise and had comparator groups performing active exercises for the same duration. Mirror therapy intervention ranged from 20 to 75 minutes per session, two to five times a week for 3-8weeks. Mirror therapy interventions reduced pain, improved range of motion, strength, and function in most studies. Greater improvements for the mirror therapy groups than the comparator group were reported for pain, range of motion, and function in 20%-75% of the studies. Risk assessment for the seven studies resulted in one high, four moderate, and two low quality studies. CONCLUSIONS: Moderate quality evidence suggests that mirror therapy interventions may be effective at reducing pain, improving range of motion, strength and function for musculoskeletal hand and wrist injuries. Further investigation is warranted with larger trials with more homogenous interventions.
