What the evidence adds up to
In a quasi-experimental study of 15 amputee patients with phantom limb pain, four weeks of home-based mirror therapy produced a significant decrease in pain severity on the 0–10 Numeric Pain Intensity Scale. Patients not using a prosthesis benefited more than those who were. No significant relationship was found between the effect and demographic, amputation, or pain-related characteristics. The authors conclude mirror therapy can be used as an adjunct to medical and surgical treatment, but the sample is small and there was no randomised control group.
A 2022 follow-up to a previous randomised controlled trial in people with poststroke hemiparesis reported significant long-term improvements in occupational performance after mirror therapy, with some additional benefit in a task-based mirror therapy group. The authors state that study limitations inhibit drawing meaningful applications to the field. A 2018 conference presentation suggested that gross motor activities yield the most realistic mirror therapy protocols, but provided no quantitative outcome data.
A 2025 systematic review of mirror therapy for musculoskeletal hand and wrist injuries included seven studies with a total of 220 participants. Samples ranged from 22 to 40. All five studies that combined mirror therapy with active exercise had comparator groups doing active exercise for the same duration. Mirror therapy groups showed greater improvements than comparators for pain, range of motion, and function in 20% to 75% of the studies. Risk of bias was rated high in one study, moderate in four, and low in two. The authors conclude that moderate quality evidence suggests mirror therapy may be effective, but call for larger trials with more homogenous interventions.
A 2019 review of mirror therapy mechanisms describes the method as using optical illusion, visual feedback, and virtual reality, and notes its use in rehabilitation for dysfunction, sensation, dyskinesia, language, and pain. No original data are presented. Across all abstracts, the evidence is limited by small samples, heterogeneous protocols, and a lack of large, low-bias randomised trials that isolate the specific effect of mirror therapy from concurrent exercise. What is still missing is adequately powered trials with standardised protocols, blinded outcome assessment, and pre-specified patient stratification.

The Effect of Mirror Therapy on the Management of Phantom Limb Pain
AbstractOBJECTIVES: In the last two decades, mirror therapy has become a frequently used method of managing phantom limb pain (PLP). However, the role of nurses in mirror therapy has not yet been well defined. This study examined the effect of mirror therapy on the management of PLP, and discusses the importance of mirror therapy in the nursing care of amputee patients. METHODS: This quasi-experimental study was conducted in the pain management department of a university hospital and a prosthesis clinic in İstanbul, Turkey, with 15 amputee patients who had PLP. Forty minutes of practical mirror therapy training was given to the patients and they were asked to practice at home for 4 weeks. Patients were asked to record the severity of their PLP before and after the therapy each day using 0-10 Numeric Pain Intensity Scale. RESULTS: Mirror therapy practiced for 4 weeks provided a significant decrease in severity of PLP. There was no significant relationship between the effect of mirror therapy and demographic, amputation or PLP-related characteristics. Patients who were not using prosthesis had greater benefit from mirror therapy. CONCLUSION: Mirror therapy can be used as an adjunct to medical and surgical treatment of PLP. It is a method that patients can practice independently, enhancing self-control over phantom pain. As mirror therapy is a safe, economical, and easy-to-use treatment method, it should be considered in the nursing care plan for patients with PLP.

Effectiveness of mirror therapy to treat musculoskeletal injuries of the hand and wrist: A systematic review
AbstractBACKGROUND: Mirror therapy is an effective intervention for improving outcomes when an affected extremity has severe movement restrictions or pain. PURPOSE: To investigate the effects of mirror therapy interventions on musculoskeletal injuries of the hand/wrist. STUDY DESIGN: Systematic review. METHODS: This systematic review was conducted in accordance with Cochrane and PRISMA guidelines and registered with Open Science Framework (DOI:XXX). The search was conducted in EMBASE, PubMed, and Scopus in April 2024. Search terms included hand and wrist injuries, mirror therapy, pain, range of motion, strength, and function. Descriptive synthesis was used to summarize the data for interventions and outcomes. The Cochrane RoB 2 tool was used for quality assessment. RESULTS: Samples ranged from 22 to 40 participants with a total of 220 participants. Five studies combined mirror therapy with active exercise and had comparator groups performing active exercises for the same duration. Mirror therapy intervention ranged from 20 to 75 minutes per session, two to five times a week for 3-8weeks. Mirror therapy interventions reduced pain, improved range of motion, strength, and function in most studies. Greater improvements for the mirror therapy groups than the comparator group were reported for pain, range of motion, and function in 20%-75% of the studies. Risk assessment for the seven studies resulted in one high, four moderate, and two low quality studies. CONCLUSIONS: Moderate quality evidence suggests that mirror therapy interventions may be effective at reducing pain, improving range of motion, strength and function for musculoskeletal hand and wrist injuries. Further investigation is warranted with larger trials with more homogenous interventions.

Mirror therapy, an important but underappreciated rehabilitation treatment
AbstractMirror therapy is a treatment method that uses flat-panel imaging combined with rehabilitation training programs to allow patients to observe the motion of the unaffected limbs, and to visualize and drive the motion of the limb on the affected side through optical illusion, visual feedback, and virtual reality. The development of mirror therapy has been widely used today in all aspects of rehabilitation programs, such as the recovery of dysfunctions, sensation, dyskinesia and language, especially the improvement of pain and exercise capacity. This review focused the mechanisms of mirror therapy, reported the possible mechanisms that may involve in the therapy.
