What the evidence adds up to
A 2015 pilot study in north-east India tested a specific intervention to improve treatment adherence in 30 patients with mental illness. Using the Medication Adherence Rating Scale and a structured Treatment Adherence Checklist, the study found a statistically significant improvement in adherence as reported by patients (paired t = 3.973, P = 0.00) and by family members (paired t = 2.94, P = 0.00) one month after the intervention. The authors caution that the findings might be generalised only after testing in a larger sample.
A 1979 survey of an institution for mentally retarded children found that 60% of residents were on drug therapy, 47% of those on psychotropic drugs. 65% of medicated residents received two or more drugs, and 78% of prescriptions had been written within the previous two years. The authors concluded that drug therapy, not behaviour modification, was the treatment of choice for behaviour problems in that setting.
A 1997 review of treated outcomes in severe and enduring mental disorders cites evidence for the efficacy of medications and for the effectiveness of family interventions, assertive community treatment, skills training, and supported employment. It directs future research to the longer-term impact of both somatic and psychosocial treatments.
A 2022 editorial notes that neuropsychiatric disorders impair millions globally but that much remains unknown about the underlying molecular mechanisms. It states that identifying these mechanisms may lead to the rational design of targeted treatments. What is still missing is the basic molecular understanding needed to design those treatments, along with the larger trials and longer follow-up required to test any intervention beyond pilot scale.

Pramipexole augmentation in the treatment of unipolar and bipolar depression: a retrospective chart review.
AbstractOBJECTIVE: To assess the effectiveness and safety of pramipexole as an adjunctive medication in refractory bipolar and unipolar depression in a naturalistic setting. METHODS: Retrospective chart review by psychiatrists on staff at a university hospital identified all patients who had received pramipexole. Response was based on moderate to marked improvement in the Clinical Global Impression-Improvement (CGI-I) scale. RESULTS: Pramipexole (mean dose 0.70 mg/d, mean duration 24.4 weeks) was effective in 6/12 (50.0%) of patients with bipolar depression, and 8/20 (40%) of patients with unipolar depression, mean duration of follow-up of 24.4 weeks. One case of transient hypomania was noted. Eight patients discontinued pramipexole due to lack of response and four due to side effects. CONCLUSIONS: Pramipexole, used as an adjunct to antidepressants or mood stabilizers, appeared to be effective and safe in the treatment of unipolar and bipolar depression. These uncontrolled, retrospective, naturalistic pilot data require confirmation by controlled research before conclusions can be made.
An Evaluation of Chlorpromazine (“Largactil”) in Psychiatry
AbstractDuring the last two years, considerable interest has been aroused in the use of chlorpromazine in psychiatry, particularly on the Continent. A full report on the pharmacological properties of the drug appeared in 1953 (Courvoisier et al. ), and a short account was given in the first paper describing the effects of chlorpromazine in the treatment of mentally ill patients in this country (Anton-Stephens, 1954). In that study, the drug was administered to patients in several diagnostic categories. A reduction in the disturbed behaviour of excited patients was observed, but considerable variation in response to the drug occurred. Small doses orally were ineffective, and a parenteral daily dosage of 200 mgm. was initially required to produce a clinical response.

Effectiveness of specific intervention on treatment adherence of persons with mental illness: A pilot study
AbstractCONTEXT: Treatment nonadherence is one of the major obstacles in recovery even with the availability of a broad range of pharmacological and psychosocial treatments for persons with mental illnesses. AIMS: The aim was to evaluate the effectiveness of specific interventions in improvement of treatment adherence of persons with mental illnesses. SETTINGS AND DESIGN: A quasi-experimental study was conducted at a tertiary mental health care setting in North-east India. MATERIALS AND METHODS: Total 30 numbers of patients were selected randomly for the study. Treatment adherence was assessed using the Medication Adherence Rating Scale and a structured Treatment Adherence Checklist. Data were collected before and 1-month after the specific interventions to the patients and their family members. RESULTS: Result showed a significant improvement in the treatment adherence as verbalized by the patient (paired t = 3.973, P = 0.00, df = 29) as well as reported by the family members (paired t = 2.94, P = 0.00, df = 29) following the specific intervention. CONCLUSION: The study result suggested that specific intervention may be used for a better outcome of treatment for mental illnesses. The findings might be generalized following implementation of the intervention to a larger sample.
Prevalence of Drug Therapy for Institutionalised Mentally Retarded Children
AbstractSummaryThe prevalence of drug therapy for behaviour problems and emotional disorders was investigated for retarded persons residing in an institution for the mentally retarded. Sixty percent of the residents were on drug therapy and of these, 47 percent were on psychotropic drugs. Sixty-five percent of those on medication received two or more drugs and 78 percent of the drugs had been prescribed within two years of the survey date. The high proporton of patients on medication for behaviour problems indicated that drug therapy, and not behaviour modification, is the treatment of choice.

Editorial: Molecular mechanisms of neuropsychiatric diseases
AbstractNeuropsychiatric disorders, such as depression, schizophrenia, bipolar disorder, obsessive 29 compulsive disorder, post-traumatic stress disorder (PTSD), autism spectrum disorder and others are 30 estimated to impair millions of individuals globally. Despite the high global prevalence, much 31 remains unknown about the underlying molecular mechanisms that lead to these and other 32 neuropsychiatric diseases. The likelihood of developing these disorders may depend on a variety of 33 genetic, developmental and environmental factors. The molecular alterations produced by these 34 factors may contribute to both disease onset or the persistence of a specific pathology. Identifying the 35 molecular mechanisms of a disease will increase our understanding of the underlying 36 pathophysiology and ultimately lead to the rational design of targeted treatments. The goal of this 37 Research Topic was to explore fundamental molecular mechanisms underlying neuropsychiatric 38 diseases, and how these mechanisms may be exploited for potential therapeutic benefit.
Efficacy and safety of long-term cabergoline treatment of antipsychotic-induced hyperprolactinemia (naturalistic study)
AbstractIntroduction Antipsychotic-induced hyperpolactinemia (AIH) is associated with disturbing clinical symptoms, such as sexual dysfunctions, menstrual disorders and galactorrhea. Long-term studies of dopamine agonists in AIH are scarce. Objectives To assess efficacy and safety of cabergoline use in psychiatric patients with AIH, including impact on sexual function and quality of life(QoL). Methods It was an open-labeled non-randomized naturalistic prospective comparison of cabergoline vs nо treatment in 84 chronic psychiatric patients (F/M = 77/7) with AIH. Cabergoline treatment was started in 44 patients, the сontrol group included 40 patients who rejected the treatment with cabergoline. For assessment of QoL, sexual dysfunction and other hyperprolactinemia symptoms, UKU side effects rating scale (UKU) and SF-36 were used. Results The main and control groups were comparable on all main clinical and psychiatric characteristics. The effective cabergoline dose was 0,25-3 (median-0,5) mg weekly; total cabergoline exposure–534 patient-weeks. Normal prolactin levels were achieved after 4-44 (median-14) weeks in 95% of patients. At 3 months after cabergoline discontinuation, prolactin remained normal in 71%, and AIH recurred in 29% of patients. Prolactin normalization was associated with significant reduction of menstrual disorders, galactorrhea, improvement of UKU scores on sexual desire, orgastic dysfunction, total UKU score and SF-36 scores on subscales of social functioning ( P = 0,006), role-emotional ( P = 0,042), and mental health ( P = 0,049). The rate of psychosis exacerbation in control group was higher than in the treatment group (37,5% vs 0%; P < 0,001). Conclusions Cabergoline is effective and safe in majority of AIH patients. Long-term cabergoline treatment is not associated with psychosis exacerbation. Beyond reversal of typical AIH symptoms, treatment with cabergolin improves sexual function and QoL.
