What the evidence adds up to

Giant-cell carcinoma of the lung is usually rapidly fatal, and nearly all published cases up to 1969 were described from postmortem findings. In that year, three surgical specimens from resected giant-cell carcinomas were described, and the patients’ subsequent courses summarised; a further nine surgically treated cases were found in the literature. The tumour was thought to be an anaplastic variant of adenocarcinoma, and its aggressive behaviour a reflection of its degree of anaplasia. Despite that behaviour, patients could survive for many years if the tumour was resected.

A 2022 study examined real-world treatment patterns and outcomes among patients with advanced non-small-cell lung cancer with spindle cell and/or giant cell carcinoma. The supplemental material for that study is available but the abstract itself does not report any numerical results, response rates, or survival data. No drug names, treatment regimens, or efficacy comparisons are given in the abstract.

A 2015 review focused on maintenance therapy for small cell lung cancer, not giant cell carcinoma. It noted that small cell lung cancer is highly sensitive to first-line chemotherapy but most patients relapse quickly after first-line therapy, and that despite many clinical trials of chemotherapy, biological agents, and molecular targeting agents since the 1980s, there remains a substantial lack of consensus on appropriate maintenance therapy. This review does not address giant cell carcinoma.

What is still missing is any prospective trial specifically for advanced giant cell carcinoma of the lung, any data on modern systemic therapy outcomes in that histology, and any patient stratification beyond surgical resectability. The 2022 study’s full results may address some of these gaps, but the abstract alone does not.

Pathology and survival in operable cases of giant-cell carcinoma of the lung

AbstractGiant-cell carcinoma of the lung is usually rapidly fatal and nearly all of the published cases have been described on the basis of postmortem findings. The surgical specimens from three resected giant-cell carcinomas are described and the subsequent courses of the patients are summarized. A further nine surgically treated cases have been found in the literature. Giant-cell carcinoma of the lung is thought to be an anaplastic variant of adenocarcinoma and its aggressive behaviour is a reflection of its degree of anaplasia. Despite the aggressive behaviour of this type of cancer, patients may survive for many years if the tumour is resected.

Small cell lung cancer is highly sensitive to first-line chemotherapy, but most of the patients relapse after the first-line therapy quickly. Despitemany clinical researchof chemotherapy, biological agents and molecular targeting agents since the 1980s, there still remains a substantial lack of consensus regarding the appropriate therapeutic management on maintenance therapy of small cell lung cancer. The paper, originally published in the Journal of Thoracic Oncology, comprehensively analysed the effect of afatinib treatment in 693 patients with non-small cell lung cancer harboring uncommon EGFR mutations. The results provide important information when considering optimal treatment for tumors with uncommon EGFR mutations, including compound mutations.

Combination Chemotherapy With or Without Maintenance Sunitinib Malate (NSC 736511) for Untreated Extensive Stage Small Cell Lung Cancer: A Phase IB/Randomized Phase II Study

AbstractThis partially randomized phase I/II trial studies the side effects and best dose of sunitinib malate and to see how well it works when given together with cisplatin or carboplatin and etoposide in treating patients with extensive-stage small cell lung cancer. Drugs used in chemotherapy, such as cisplatin, carboplatin, and etoposide, work in different ways to stop the growth of tumor cells, either by killing the cells or by stopping them from dividing. Sunitinib malate may stop the growth of tumor cells by blocking some of the enzymes needed for cell growth and by blocking blood flow to the tumor. It is not yet known whether cisplatin or carboplatin and etoposide are more effective when given with or without sunitinib malate in treating small cell lung cancer.
