What the evidence adds up to

Solitary fibrous tumour of the liver is a rare mesenchymal tumour, with only about 50 cases reported in the liver parenchyma by 2015. Radiological findings are not specific and cannot confirm whether a tumour is benign or malignant. Immunohistological markers such as CD34, Vimentin, and Desmin are used for precise diagnosis. Most cases show low cellularity with no cellular atypia or necrosis and are considered benign. Occasionally, large size, high mitotic rate, cellular pleomorphism, atypia, and necrosis are interpreted as features suggestive of increased malignant potential. The tumour can be malignant and can metastasise.

The outcome is mostly related to resectability, and is correlated with neither pathological grade nor tumour size. Total surgical resection is the most commonly proposed treatment. In one 2020 case report, a 54-year-old male patient with primary liver SFT and additional tumours in the spleen and chest wall underwent two operations to remove all tumours. Six years later, the tumour recurred in the liver and was too large for complete surgical resection. The patient received orthotopic liver transplantation and had no tumour recurrence during 12 months of follow-up. A 37-year-old female patient with a primary splenic vein SFT experienced repeated recurrence after surgical resection over 10 years, with eventual transfer to the liver, but was in good condition after four operations.

A 2015 case report described a patient whose imaging data from five years before diagnosis were available; that patient was treated with right portal embolisation and arterial tumour embolisation followed by liver resection. No controlled trials or prospective studies exist for this disease. What is still missing is any prospective clinical trial, any systemic therapy data, and any reliable method to stratify patients by malignant potential before surgery. The evidence base remains limited to case reports and small retrospective series.

Solitary Fibrous Tumor of the Liver: Retrospective Study of Reported Cases

AbstractA solitary fibrous liver tumor is a rare disease that is difficult to diagnose. Radiological findings are not specific and cannot confirm benignity or malignancy. Immunohistologically, CD 34, Vimentin, and Desmin should be used as markers to precisely diagnose solitary fibrous tumors. In most cases, there is low cellularity with no cellular atypia or necrosis, making this a benign lesion. Occasionally, a large size, high mitotic rate, cellular pleomorphism and atypia, and necrosis are interpreted as features suggestive of an increase malignant potential. Solitary fibrous tumor of the liver. Case report and review of the literature

AbstractSolitary fibrous tumor (SFT) is a rare mesenchymal tumor. Given its origin, it can appear in almost any location. In the literature, only 50 cases of SFT in the liver parenchyma have been reported. Despite its rarity, this entity should be included in the differential diagnosis of liver masses. We report the first case with imaging data from five years prior to diagnosis, which was treated by right portal embolization and arterial tumor embolization, and subsequent liver resection. We also present an exhaustive review of the cases described to date.

Two case reports of rare diseases occurring in rare parts: Splenic vein solitary fibrous tumor and Liver solitary fibrous tumor

AbstractAbstract Background Solitary fibrous tumor (SFT) is a rare soft tissue tumor originating from mesenchyme. Two cases of SFT we report right now occurred in the splenic vein and liver respectively, this primary splenic vein SFT may be the first report case, and also the first report of liver recurrence SFT cured by orthotopic liver transplantation (OLT). Case presentation One case was a 37-year-old female patient whose primary tumor site was located in the splenic vein, which resulted in splenomegaly and hypersplenism; its recurrence again and again after surgical resection and eventually transferred to the liver, during 10 years of follow-up, 4 operations were performed, and he is in a good condition right now. The second case was a 54-year-old male patient whose primary tumor site was located in the liver, spleen and left side of the chest wall; however, he had no uncomfortable symptoms. Surgeons performed two operations to remove these tumors, totally. 6 years later, SFT recurrence in the liver, and given that the tumor was so large that it could not be completely surgical resected, we chose orthotopic liver transplantation (OLT), and no tumor recurrence during 12-month follow-up. Conclusion The reports of these two cases of SFT are very rare, especially the splenic vein SFT, which expand the understanding of SFT. The main treatment of SFT is still surgical resection, right now, and liver transplantation may be a new option treatment for the huge liver SFT.

Case Report: Successful Treatment of Solitary Fibrous Tumor with Selective Internal Radiation Therapy (SIRT)

AbstractBackground: This case report details the innovative use of selective internal radiation therapy (SIRT) with Yttrium-90 resin microspheres to treat a 73-year-old woman with a solitary fibrous tumor (SFT), a rare and challenging tumor type. SFTs often present significant treatment difficulties, especially in cases of recurrence or metastasis, as systemic therapies typically show limited effectiveness. This report explores SIRT as an alternative therapeutic approach for SFTs with liver metastasis. Methods: The patient initially presented with a pelvic mass, which was surgically resected. However, metastatic disease later developed in the liver. After experiencing severe side effects from targeted therapy with sunitinib, the patient was selected for treatment with SIRT as an alternative. Results: Following the SIRT intervention, the patient demonstrated a substantial reduction in tumor size and significant relief from symptoms. This outcome suggests SIRT’s effectiveness as a targeted treatment for metastatic SFT. Conclusions: To our knowledge, and based on an extensive literature review, this is the first reported instance of treating SFT with SIRT. This case provides new insights into SIRT’s potential as a therapeutic strategy, particularly for patients for whom conventional treatments are either ineffective or intolerable. The success observed here underscores SIRT’s potential as a less invasive, locally targeted treatment option, offering hope for similar cases.
