What the evidence adds up to

Intestinal pseudo-obstruction is a syndrome with many causes, and treatment must be guided by the specific cause and extent of gastrointestinal involvement in each patient. For primary chronic cases, a family history should be taken; if positive, genetic counselling and proper medical management can be offered to newly identified family members.

Cisapride has been used in chronic intestinal pseudo-obstruction with some success, but its use in acute pseudo-obstruction had not been adequately studied as of 2003. One case report describes a patient whose acute intestinal pseudo-obstruction was probably precipitated by pneumonia and sepsis. After initiation of cisapride therapy, the pseudo-obstruction resolved promptly, even as the patient’s overall clinical status progressively deteriorated. The authors implicate cisapride as the cause of improved intestinal function and suggest a trial of cisapride be considered for acute pseudo-obstruction when other therapeutic manoeuvres have failed. This is the second such case reported in the English language literature.

No controlled trials, no randomised data, and no quantitative outcomes such as survival or response rates are provided in these abstracts. The evidence for cisapride in acute pseudo-obstruction rests on two single-case reports. What is still missing is any prospective trial design, adequate sample sizes, and patient stratification by underlying cause.

Clinical Recovery of Chronic Intestinal Pseudo-Obstruction With Cisapride in a Complex Pediatric Patient

AbstractCisapride is a gastrointestinal prokinetic that facilitates or restores motility along the entire gastrointestinal tract. It has been used successfully to treat acute and chronic intestinal pseudo-obstructions (CIPs) in adults, but there is a paucity of literature surrounding the treatment of CIP in pediatric patients and therapies for CIP are limited and their impact is often unsatisfactory. This case report presents the use of cisapride in the management of pseudo-obstruction. Treatment with cisapride substantially improved the patient's symptoms and improved feeding tolerance. It improved his prognosis remarkably and prevented the need for end-of-life care. He experienced no adverse effects throughout the course of therapy. The treatment regimen is discussed in this case report.

AbstractCisapride has been used in the treatment of chronic intestinal pseudo-obstruction with some success; however, the use of cisapride in the treatment of acute pseudo-obstruction has not been adequately studied. We describe a case of acute intestinal pseudo-obstruction successfully treated with cisapride. To our knowledge, this is the second such case reported in the English language literature. Our patient's acute intestinal pseudo-obstruction was probably precipitated by pneumonia and sepsis, and the prompt resolution of his intestinal pseudo-obstruction after initiation of cisapride therapy (despite progressive deterioration of his overall clinical status) implicates cisapride as the cause of the improved intestinal function. Therefore, we suggest that a trial of cisapride be considered for patients with acute intestinal pseudo-obstruction when other therapeutic maneuvers have failed.
