DOCK8 immunodeficiency syndrome (DIDS) is a combined immunodeficiency marked by recurrent viral infections, severe atopy, and early onset malignancy. Genetic studies found large, unique deletions in patients from different families and ethnic backgrounds. Clinical markers include atopic dermatitis, allergies, cutaneous viral infections, recurrent respiratory tract infections, and malignancy. Immune assessments showed T cell lymphopenia, hyper-IgE, hypo-IgM, and eosinophilia. Impaired lymphocyte functions appear central to disease pathogenesis.
Common variable immunodeficiency (CVID) is a heterogeneous group of primary antibody deficiency. Patients have increased susceptibility to infections and low serum immunoglobulin, but enteropathy, granulomatous organ infiltrates, malignancy, inflammatory and autoimmune conditions are also prevalent. The coexistence of immunodeficiency and autoimmunity creates difficulties in management because immune dysregulation and persistent inflammation impair diagnosis and treatment. Among combined immunodeficiency disorders, the highest incidence of autoimmunity is described in autoimmune polyendocrine syndrome 1, LRBA deficiency, CTLA-4 deficiency, and STAT-related disorders. The pathogenesis of autoimmunity in immunodeficiency is not fully elucidated, but altered germinal centre reactions, impaired lymphocyte negative selection, uncontrolled lymphocyte proliferation, ineffective cytoskeletal function, innate immune defects, and defective clearance of infectious agents play a role.
For patients with significant antibody deficiencies, immunoglobulin therapy is the mainstay of treatment and significantly reduces both the frequency and severity of infections. Formulations and delivery methods have evolved to increase access. In HIV infection, chronic immune activation with progressive immune exhaustion are central features; the role of self-reactive T cells in generating and maintaining this process has been described, but the impact of immune dysregulation on autoimmune phenomena in HIV remains incompletely understood. The cause, frequency and prognosis of autoimmune conditions associated with HIV are somewhat uncertain, and management is often empirical with novel immunosuppressive medication.
What is still missing are targeted treatments for the specific genetic defects in DOCK8 deficiency and other combined immunodeficiencies with autoimmunity, as well as prospective trials that stratify patients by underlying genetic cause rather than by clinical phenotype alone. The diagnostic algorithm proposed for primary immunodeficiencies in patients with autoimmunity has not been validated in large, diverse populations. Funding for mechanistic studies linking specific immune defects to autoimmune manifestations, and for trials of therapies that do not further impair host defence, remains inadequate.

Autoimmunity in common variable immunodeficiency: epidemiology, pathophysiology and management
AbstractINTRODUCTION: Common variable immunodeficiency (CVID) comprises a large heterogeneous group of patients with primary antibody deficiency. Areas covered: The affected patients are characterized by increased susceptibility to infections and low levels of serum immunoglobulin. However, enteropathy, granulomatous organ infiltrates, malignancy, inflammatory and autoimmune conditions are also prevalent. The concomitance of immunodeficiency and autoimmunity appears to be paradoxical and creates difficulties in the management of autoimmune complications affecting these patients. Expert commentary: The management of autoimmunity in patients with CVID requires special considerations because dysregulation and dysfunctions of the immune system along with persistent inflammation impair the process of diagnosis and treatment.

An Update on the Use of Immunoglobulin for the Treatment of Immunodeficiency Disorders
AbstractFor patients with significant antibody deficiencies, immunoglobulin therapy is the mainstay of treatment as it significantly reduces both the frequency and severity of infections. The formulations and delivery methods of immunoglobulin have evolved over time, and continued improvements have allowed for increased access to this effective medication. This review is an update on the current status of immunoglobulin therapy in immunodeficiency disorders, and discusses the mechanisms, forms and dosing, and indications for immunoglobulin replacement.

Autoimmunity in Primary Immunodeficiency Disorders: An Updated Review on Pathogenic and Clinical Implications
AbstractDuring the last years, studies investigating the intriguing association between immunodeficiency and autoimmunity led to the discovery of new monogenic disorders, the improvement in the knowledge of the pathogenesis of autoimmunity, and the introduction of targeted treatments. Autoimmunity is observed with particular frequency in patients with primary antibody deficiencies, such as common variable immunodeficiency (CVID) and selective IgA deficiency, but combined immunodeficiency disorders (CIDs) and disorders of innate immunity have also been associated with autoimmunity. Among CIDs, the highest incidence of autoimmunity is described in patients with autoimmune polyendocrine syndrome 1, LRBA, and CTLA-4 deficiency, and in patients with STAT-related disorders. The pathogenesis of autoimmunity in patients with immunodeficiency is far to be fully elucidated. AbstractPURPOSE OF REVIEW: This review focuses on some of the recent advances in the understanding of HIV immunopathogenesis and the diagnosis and treatment of several autoimmune conditions associated with HIV in the era of potent antiretroviral therapy. RECENT FINDINGS: Chronic immune activation with progressive immune exhaustion are central features of HIV pathogenesis. The role of self-reactive T cells in the generation and maintenance of this process has recently been described. The understanding of the impact of immune dysregulation on the generation of autoimmune phenomena in HIV infection remains incomplete. The diagnosis of autoimmune diseases in the context of HIV is often difficult due to similarities in clinical presentations and laboratory markers. The antiretroviral therapy-associated immune reconstitution syndrome can present as autoimmune disease. SUMMARY: The cause, frequency and prognosis of autoimmune conditions associated with HIV infection remain somewhat uncertain. Their management is often empirical with the use of novel immunosuppressive medication.
AbstractThere are approximately 35 million people infected by human immunodeficiency virus (HIV), with an estimated 2 million incident infections annually across the globe. While HIV infection was initially associated with high rates of morbidity and mortality, advances in therapy have transformed it into a chronic and manageable disease. In addition, there is very strong evidence that those on antiretroviral therapy are much less likely to transmit infection to their partners. The success rates for maintaining viral suppression in treated patients has dramatically increased owing to the development of agents that are potent and well tolerated and can often be co-formulated into single pills for simplification. This review will outline advances in treatment over the last several years as well as new strategies that may shift the existing treatment paradigm in the near future.

AbstractABSTRACT: Human immunodeficiency virus (HIV) is widely prevalent among the world population. Although, historically, it has been linked to opportunistic infections in keeping with immunodeficiency and immune dysregulation, it has also been associated with a wide variety of autoimmune manifestations. With the introduction of highly active antiretroviral therapy and subsequent restoration of immunity, there have been multiple immune-mediated diseases that have resurfaced in the HIV population. Our review highlights autoimmune diseases in association with HIV and its targeted therapies in detail.
