What the evidence adds up to
In a 1978 review of 39 chemotherapy trials in 16 patients with metastatic hemangiopericytoma at Memorial Sloan Kettering, plus 33 trials from the literature, adriamycin (doxorubicin) alone or in combination produced complete and partial remission in 50% of cases. Other drugs with some reported activity included vincristine, cyclophosphamide, actinomycin, methotrexate and DTIC. The authors concluded that malignant hemangiopericytoma should be considered chemotherapeutically responsive. A 1974 report from the National Cancer Institute of Milan on 11 cases noted that surgical excision played a very important role in management and that palliative results had recently been obtained with chemotherapy, but gave no response rates or survival numbers.
A 2020 case report described a 29-year-old pineoblastoma patient who developed an intracranial hemangiopericytoma at a different location four years after successful surgical excision and adjuvant radiotherapy for the original tumour. The authors concluded that hemangiopericytoma emergence can be seen after radiotherapy. A 2024 case report described a patient with disseminated hemangiopericytoma treated with radiation alone, emphasising that careful use of different radiotherapy techniques in tandem allowed long survival and preserved quality of life, but this is a single case and no survival duration or tumour response measurements were given.
The evidence base remains limited to small retrospective series and single case reports. No prospective trial has defined optimal chemotherapy, radiotherapy dose-fractionation, or sequence of modalities. The 1978 response rate of 50% to adriamycin-based regimens has not been replicated in a modern, larger cohort. What is missing is a multi-centre registry or randomised trial that stratifies patients by histological grade, mutational profile, and extent of disease, and that measures progression-free survival and quality of life with standardised criteria. Funding for such a trial has not been reported.

AbstractThirty-nine chemotherapy trials in 16 patients with metastatic hemangiopericytoma treated at MSKCC and 33 trials from the literature are reviewed. Adriamycin, alone or in combination drug regimens, is the most effective agent, producing complete and partial remission in 50% of cases. Other drugs which show some activity include vincristine, cyclophosphamide, actinomycin, methotrexate and DTIC. The available data indicate malignant hemangiopericytoma should be considered chemotherapeutically responsive tumors.

AbstractThe main features of the hemangiopericytoma: its ubiquity, the different grades of malignancy, the lack of correlation between clinical and histological features are discussed in the light of the literature and the experience gained at the National Cancer Institute of Milan. Surgical excision plays a very important role in the management of this tumor. The palliative results recently obtained with chemotherapy are set forth.

AbstractBACKGROUND: Radiation exposure is a known risk factor for meningioma but there are no data regarding hemangiopericytoma and radiation exposure. CASE DESCRIPTION: We report a 29-year-old pineoblastoma patient diagnosed with a hemangiopericytoma at a different location, after a successful surgical excision and adjuvant radiotherapy for the original tumor 4-year prior. CONCLUSION: Hemangiopericytoma emergence can be seen after radiotherapy.

Long and Meaningful Survival of a Case with Disseminated Hemangiopericytoma - Treated with Radiation Alone
AbstractHemangiopericytomas are tumours arising from the pericytes of Zimmerman. They arise in the central nervous system and have a locally recurrent behaviour with occasional distant metastases too. This case highlights the careful and judicious use of different radiotherapy techniques used in tandem to ensure a long survival for a patient. Emphasis is made on the quality of life as he was able to function to his maximum till the end.
