What the evidence adds up to

In 2013, the first laboratory-confirmed cases of hantavirus infection from the Middle Anatolia Region of Turkey were reported, including the first documented co-infection of hantavirus and Crimean-Congo hemorrhagic fever virus in a single patient. That co-infected patient was tested for hantavirus only because the illness lasted longer than expected for Crimean-Congo hemorrhagic fever alone and because renal function was impaired. Diagnosis of Crimean-Congo hemorrhagic fever was confirmed by serological and molecular methods, and IgM and IgG antibodies for hantavirus were also positive. All three patients in this report were discharged after receiving supportive therapy only; no antiviral or specific drug treatment was described.

A 2012 case report describes a patient in South Korea with hemorrhagic fever with renal syndrome and coexisting hantavirus pulmonary syndrome, a combination rarely seen outside the Americas. The report does not provide any drug treatment or survival outcome beyond noting the presentation of both syndromes. A 2009 review states that the pathogenesis of hantavirus is not yet entirely clear, listing virus, virus receptor, cytokines, free radicals, specific CTL response, and HLA differences as possibly related factors, but offers no therapeutic intervention.

A 2019 study evaluated the activity of human serum albumin on selected hantavirus aptamers, focusing on diagnostic mechanisms rather than treatment. No drug, antiviral, or therapeutic candidate is tested or proposed in any of these abstracts. The evidence base for treating hantavirus hemorrhagic fever with renal syndrome remains limited to supportive care, with no controlled trial data, no repurposed drug tested in patients, and no validated animal model results reported here. What is still missing is any randomised clinical trial, any drug candidate with demonstrated human efficacy, and any clear patient stratification strategy for future studies.

The co-infected patient was tested for hantavirus infection because the duration of the illness lasted longer than expected for CCHF and renal function was impaired. Diagnosis of CCHF was confirmed with serological and molecular methods; in addition, IgM and IgG antibodies for hantavirus were positive. All three patients were discharged after they received supportive therapy.

AbstractHemorrhagic fever with renal syndrome (HFRS) is an acute viral disease with fever, hemorrhage and renal failure caused by hantavirus infection. Hantavirus induces HFRS or hantavirus pulmonary syndrome (HPS). HPS progression to a life-threatening pulmonary disease is found primarily in the USA and very rarely in South Korea. Here, we report a case of HFRS and coexisting HPS.

Evaluation of HSA activity on selected hantavírus aptamers

AbstractHantaviruses are enzootic viruses that maintain persistent infections in their rodent hosts without apparent disease symptoms. The spillover of these viruses to humans can lead to serious illnesses as, hantavirus pulmonary syndrome and hemorrhagic fever with renal syndrome. In recent years, there has been an improved understanding of the epidemiology, pathogenesis, and natural history of these viruses following by the necessity to investigate a diagnostic mechanism.
