What the evidence adds up to
A 2019 study of 62 patients (124 eyes) with visual display terminal syndrome randomly assigned 31 patients to artificial tears alone and 31 to artificial tears plus auricular acupressure, tuina around the orbits, and whole-body tuina, with health education given to both groups. After two weeks, symptomatic scores and facility of accommodation improved significantly in both groups compared to before treatment. Amplitude of accommodation and near point of convergence improved significantly only in the group that received the additional acupressure and tuina. The study did not report survival or response rates because the condition is not life-threatening; it measured symptom scores and accommodative function.
A 2022 analysis of 58 patients (58 eyes) with hyperopia found accommodation disorders in all of them: constantly redundant tension of accommodation in 12.1%, weakness of accommodation in 70.7%, and a combination of both in 17.2%. The authors noted that accommodation indices depend on patient age, which must be accounted for when comparing groups. No treatment was tested in this analysis.
A 1980 review categorised ocular difficulties in near gaze into disorders of accommodation and convergence, ocular muscle paralyses, supranuclear gaze disorders, and restrictive ocular disease, and described differential diagnosis and management suggestions. It did not report any numerical outcomes.
What is still missing: no randomised trial has tested any drug specifically for eye accommodation disease. The 2019 study tested physical therapies, not drugs. No trial has stratified patients by the type of accommodation disorder (weakness, tension, or combination) or by age, which the 2022 analysis shows matters. Funding for a proper drug trial, a clear definition of the target patient population, and a validated primary endpoint beyond symptom scores are all absent.

Ocular Difficulties in the Neargaze Position
AbstractThe clinical entities which contribute to ocular symptoms in the near position have been divided into categories. These include disorders of accommodation and convergence, ocular muscle paralyses, supranuclear disorders of gaze, and restrictive ocular disease. Patients may exhibit clinical features from one or more of these groups. Differential diagnostic methods are described, and suggestions for management of the ocular symptoms are given.

Clinical study on visual display terminal syndrome with local treatment near the eyes in combination with systemic comprehensive treatment
AbstractAIM: To research the clinical effects of visual display terminal(VDT)syndrome after local treatment near the eyes in combination with systemic comprehensive treatment. METHODS: Totally 62 patients(124 eyes)with VDT syndrome were randomly divided into a trial group and a control group, 31 cases(62 eyes)in each group. In the control group, artificial tears as eye drops were given. In addition to the above local treatment with artificial tears, the trial group was also treated with the auricular acupressure and tuina at the acupoints near orbits and the whole body. A health education was carried out in both of the two groups. The course of the treatment of the two groups was 2wk. The symptomatic scores, and the amplitude of accommodation, facility of accommodation, accommodative response, near point of convergence and the ratio of accommodative convergence to accommodation(AC/A)based on refractive error correction of all the patients were detected before and after the treatment. The parameters gained before and after the treatment were compared for evaluating efficacy. RESULTS: After treatment, the symptomatic scores and facility of accommodation of the two groups were significantly improved compared with those before treatment(<i>P</i><0.05). Amplitude of accommodation and near point of convergence in the trial group were significantly improved after treatment(<i>P</i><0.05). CONCLUSION: Local treatment near the eyes in combination with systemic comprehensive treatment can effectively improve the accommodative and convergent functions of patients with VDT syndrome, and relieve their symptoms of asthenopia and fatigue.

ANALYSIS OF ACCOMODATION STATUS IN PATIENTSWITH HYPERMETROPY
AbstractAn analysis was performed of the accommodation, hydrodynamics state of the eye, asthenopic complaints in 58patients (58 eyes) with hyperopia. Accommodation disorders were revealedin the form of constantly redundant tension of accommodation, weakness of accommodation (12,1%), weakness of accommodation (70,7 %), as well as their combination (17,2 %). The accommodation indices in patients with hypermetropydepend on the patients age that should be taken into account during performance a comparative analysis between groups.
