What the evidence adds up to
Only two medications, primidone and propranolol, have received Level A recommendations from the American Academy of Neurology for essential tremor. Up to 50% of patients are non-responders to these drugs. Many patients are not benefited adequately, and some not at all, despite numerous available treatments. The diagnosis and management of essential tremor is deceptively simple; isolated mild tremor may be difficult to classify, and additional features make diagnosis harder.
Essential tremor affects almost 1% of the population and can be very debilitating. Much remains unclear about its pathophysiology, environmental factors, and genetic causes. The diagnosis most likely encompasses several different conditions. A new tremor classification has been published, with axis 1 highlighting clinical characteristics and axis 2 dedicated to aetiology. The cerebello-thalamo-cortical network and its connections to other brain areas is emerging as pivotal to many tremor types. There has been ongoing debate around the clinical entity of essential tremor and its pathophysiological basis.
Treatment options include medications, adaptations, botulinum toxin injections, and functional neurosurgery. Increasing understanding of the pathophysiology is pushing forward trials of new treatment options, particularly surgical ones. With deeper phenotyping from the new classification, genetics of common tremor forms are ripe for discovery. New pharmacological therapeutic options are needed to complement the better understanding of the basis of tremor.
What is still missing is a clear pathophysiological model that can be targeted by drugs, adequate funding for trials that can move beyond the two approved medications, and a trial design that accounts for the likelihood that essential tremor is not a single condition but several, each possibly requiring different treatment. Patient stratification by phenotype or genotype remains an unmet need.

However, isolated mild tremor may be difficult to classify, and if the patients have any additional features the diagnosis is more difficult. Management can be challenging, despite the numerous treatments available, because so many patients are not benefited adequately and some not at all. However, as we gain a better understanding of the disorder, more effective therapies with fewer adverse effects are sure to follow.

Methodological Issues in Clinical Drug Development for Essential Tremor
AbstractEssential tremor (ET) is one of the most common tremor disorders in the world. Despite this, only two medications have received Level A recommendations from the American Academy of Neurology to treat it (primidone and propranolol). Even though these medications provide relief to a large group of ET patients, up to 50% of patients are non-responders. Additional medications to treat ET are needed. This review discusses some of the methodological issues that should be addressed for quality clinical drug development in ET.
