What the evidence adds up to

No medication is approved by the US Food and Drug Administration for eosinophilic gastroenteritis. Treatment remains empirical, based on symptom severity and clinician experience. Mild disease may be managed symptomatically; more severe cases or those with malabsorption require aggressive therapy. Options tried include dietary treatment, glucocorticosteroids, leukotriene receptor inhibitors, mast cell stabilisers, immunomodulating drugs, and biological drugs. A short course of corticosteroids is described as the treatment of choice in a 2018 case series of three patients.

The disease is rare, which limits experience with management options and prevents randomised controlled trials that could clearly delineate efficacy of new agents. A 2022 review states there is still a lack of well-designed, prospective, randomised clinical trials involving large groups of patients. The same review notes that more research is needed to compare efficacy and safety profiles of available treatments and to select prognostic factors for relapse, which would inform decisions about initial treatment and maintenance therapy.

A 2024 case report describes a 23-year-old male with recurrent abdominal pain and loose stools treated with antibiotics and antispasmodics, with no history of substance abuse, prior surgeries, comorbidities, skin allergies, breathing difficulties, or recurrent infections. The 2018 case series notes that patients may or may not have peripheral eosinophilia and that the disease has a chronic clinical course.

What is still missing are adequately funded, prospective, randomised trials enrolling sufficient numbers of patients to compare treatments head-to-head, and validated prognostic factors to stratify patients by relapse risk.

AbstractEosinophilic gastroenteritis (EG) is a rare disorder characterized by eosinophilic infiltration of the gastrointestinal tract. No medication at present is approved by the Food and drug administration of United States for the treatment of EG. The rarity of the disease limits our experience with the different management options. It also limits the ability to conduct randomized controlled trials that could clearly delineate the efficacy of new therapeutic agents. This review assesses the various management options that have been tried on patients with EG.

Eosinophilic gastroenteritis – a manifestation of an allergic disease in the gastrointestinal tract? Part 2. Treatment

AbstractTreatment of eosinophilic gastroenteritis (EGE) is mainly empirical and is based on the assessment of symptom severity and the experience of clinicians. Patients with mild disease can be treated symptomatically, while patients with more severe symptoms or malabsorption symptoms require more aggressive therapy. So far, several therapeutic options have been proposed, including the following: dietary treatment, glucocorticosteroids, inhibitors of leukotriene receptors, mast cell stabilizers, immunomodulating drugs, and biological drugs. Unfortunately, there is still a lack of well-designed, prospective. and randomized clinical trials involving large groups of patients with EGE and assessing the effectiveness of individual treatments. More research is needed to compare the efficacy and safety profiles of the various treatments available, and to select the prognostic factors of relapse, which in turn will be extremely important in making decisions about the initial treatment phase and maintenance therapy.
