What the evidence adds up to
Of 1640 women with endometrial carcinoma seen at one centre between 1993 and 2013, only 11 (0.67%) had pure mucinous type. Mean age was 55 years. Ten of the 11 (90.9%) were stage I; one was stage IIIC1. Lymph node metastasis was found in one of eight patients who had lymphadenectomy (12.5%). Four patients received adjuvant pelvic radiotherapy. Median follow-up was 50 months. One recurrence occurred, in the stage IIIC1 patient, at 30 months after primary surgery, in the upper abdomen. The authors concluded that stage I disease can be managed with surgery alone and that the prognosis is good.
A separate case report describes a patient who developed mucinous endometrial carcinoma 18 years after bilateral ovariectomy for ovarian endometrioid adenocarcinoma. The report notes that oestrogen stimulation is often postulated as a factor in such endometrial cancers, but the long interval after ovarian removal complicates that explanation.
No abstract in this set reports any drug treatment or drug response data for mucinous endometrial adenocarcinoma. The 2016 abstract on CA 19-9 as a predictor of advanced disease and nodal involvement is cut off and provides no results. The evidence base remains limited to small surgical case series and single case reports. What is missing is any prospective trial, any biomarker-validated stratification, any drug-repurposing data, and any funding for a disease this rare.

Mucınous Endometrial Cancer: Clinical Study of Eleven Cases
AbstractOBJECTIVE: In this study, we analyzed surgico-pathologic factors of mucinous type endometrial carcinoma and examined its frequency of recurrence. METHODS: In this study, eleven cases, definitely diagnosed as pure mucinous type endometrium carcinoma between January 1993 and May 2013, were reviewed. RESULTS: Of 1640 women with endometrium carcinoma, 11 (0.67%) of them had a mucinous cell type. Mean age of the study group was 55 years. According to the FIGO 2009, 10 (90.9%) cases were evaluated as stage I and 1 (9.1%) as stage IIIC1. The presence of lymph node metastasis was noticed in only one (12.5%) of eight patients who underwent lymphadenectomy. In this case, metastasis was detected in the pelvic lymph node. Four patients underwent adjuvant therapy as pelvic radiotherapy. Median follow-up time was 50 months (range, 5-84). Recurrence was observed in one (9.1%) patient with stage IIIC1 endometrial cancer in 30 months after primary surgery. The site of recurrence was only in the upper abdominal region. CONCLUSION: Based on our study, mucinous endometrial carcinoma has good prognostic factors, and long term survival can be achieved surgically alone in patients with stage I.

AbstractThe association of endometrioid carcinoma of the ovary and primary carcinoma of the endometrium is well recognised. These tumours are often synchronous in occurrence. Oestrogen stimulation is often postulated as a significant factor in the development of the endometrial carcinoma in such cases. We describe the case of a patient who developed a mucinous endometrial carcinoma 18 years after initial bilateral ovariectomy. The aetiology and pathogenesis of the uterine tumour is discussed.
