What the evidence adds up to
Eccrine porocarcinoma is a rare skin cancer. A 2016 review describes it as aggressive, noting one case of a 62-year-old man with a forearm mass that led to distant metastasis and death. The review recommends wide local resection but provides no survival statistics or response rates.
A 2015 review of surgical management reports that most cases are treated by wide local excision, which carries a 20% local recurrence rate. Among 20 cases treated with Mohs micrographic surgery, there was one nodal recurrence and no local recurrence. The authors report a 21st case successfully treated with Mohs surgery. No data on overall survival or metastasis-free survival are given for either technique.
A 2008 case report describes a 61-year-old man with a cheek mass that had grown slowly over four years and caused facial nerve palsy. He underwent radical excision with clear margins, nerve reconstruction, and postoperative radiation. Lymph nodes were negative. DNA image cytometry gave a malignancy grade of 0.73, and the MIB-1 proliferation index was below 10%. The authors recommend radical resection with clear margins and, if lymph node metastases are present, additional radiotherapy.
No controlled trials exist for eccrine porocarcinoma. The evidence is limited to case reports and small surgical series. What is missing is prospective data, standardised staging, and any systemic therapy with documented response rates.

We presented a 62-year-old male with long history of a mass in left forearm complicated by distant metastasis and death. A rapid review of literature has been presented. CONCLUSION: Eccrine porocarcinoma is a rare but aggressive type of skin disease. It may arise de novo or complicate a benign skin lesion. Wide local resection is recommended.
Mohs Micrographic Surgery for Eccrine Porocarcinoma
AbstractBACKGROUND: Eccrine porocarcinoma (EPC) is a rare malignancy of the eccrine sweat glands that is locally aggressive with a high propensity to metastasize. Most cases have been treated by wide local excision (WLE) with 20% local recurrence rate. There have been 20 cases of EPC treated with Mohs micrographic surgery (MMS) in the literature. OBJECTIVE: To review the literature regarding the management of this aggressive tumor using MMS. METHODS: A comprehensive literature review was conducted by searching the PubMed database using the keywords Mohs, porocarcinoma, malignant eccrine poroma, and eccrine neoplasms. RESULTS: Most of the reported cases of EPC were treated by WLE, and only 20 cases were treated with MMS. Of the 20 EPC cases treated with MMS, there was only 1 nodal recurrence and no local recurrence. Other reported treatment modalities include radiation and excision with frozen sections. The authors report the second case of EPC on the temple, and the 21st case successfully treated with MMS. CONCLUSION: Eccrine porocarcinoma is a rare neoplasm with potentially aggressive clinical behavior. In cases where tissue conservation is important, MMS should be considered.

AbstractBACKGROUND: Eccrine porocarcinoma is a rare malignant tumor, arising from the intraepithelial ductal parts of the sweat glands. Although the clinical appearance is variable, some tumors present with a diameter of 10 cm or more. In most cases the tumor presents as a slowly growing painless mass. PATIENTS AND METHODS: A case of a 61 year old male patient is presented, who was admitted because of a peripheral facial nerve palsy. The patient reported about a sting in this area some ten years ago with a persistent swelling. Four years ago this lesion began to grow slowly. RESULTS: Clinically a huge mass in the left cheek extended to the parotid gland was obvious. The patient underwent radical tumor excision with histologically clear margins. The defect was reconstructed by a microvascular forearmflap, the facial nerve was reconstructed using either a graft from the suralis nerve (for the upper part of the facial nerve) along with a hypoglossal facial nerve anastomosis for the lower part of the facial nerve. Postoperative radiation therapy was added. Histologically an eccrine porocarcinoma was confirmed with an infiltration of the facial nerve and the parotid gland. Lymph nodes in the neck were negative. DNA image cytometry achieved a DNA malignancy grade of 0.73, the immunohistochemical assessment using a monoclonal antibody against the proliferation antigen MIB-1 achieved a proliferation rate of < 10 %. CONCLUSIONS: Although very rare eccrine porocarcinoma should be considered in the differential diagnosis of slowly growing skin masses in the face. Therapeutically a radical resection with histological clear margins is recommended, in case of lymph node metastases with additional postoperative radiotherapy.

Porocarcinoma is commonly found in elderly persons and most common located in the lower exremities. It usually occurs on the pre- existing lesion of benign eccrin poroma. The treatment is widely local excision. Here we report of a73 years old man diagnosed with eccrin porocarcinoma that develop on the gluteal region.
