What the evidence adds up to
The DYT1 mutation is found in a minority of early-onset primary dystonia patients. In a study of 30 Italian patients with sporadic early-onset disease, 5 carried the mutation (mean age at onset 8 years), while the remaining 25 did not (mean age at onset 7.7 years). Among the DYT1-positive patients, two had the classic generalised phenotype, two had generalised dystonia involving cranial muscles, and one had segmental dystonia. In the DYT1-negative group, 22 had generalised and 3 had segmental dystonia. German families with the DYT1 mutation have shown that writer's cramp or focal cervical dystonia can also be presentations, not only generalised dystonia.
Neuropathology differs between early-onset and adult-onset primary dystonia. In six cases of adult-onset dystonia, no torsinA-, ubiquitin-, or laminA/C-immunoreactive neuronal inclusions were found in brainstem nuclei, and there was no loss of the striatal striosome compartment. This contrasts with early-onset DYT1-related dystonia, where such inclusions have been reported, and with DYT3 X-linked dystonia-parkinsonism, where striosomal neuronal loss occurs. The underlying mechanism in adult-onset primary dystonia therefore appears to be distinct from these genetic forms.
The most common dystonias are adult-onset focal forms, but their causes remain poorly understood compared with rarer generalised genetic dystonias. A 2013 review notes that knowledge of adult-onset focal dystonia aetiology and pathogenesis has lagged behind, and outlines the need for further study of less well understood clinical manifestations. A 2018 review describes dystonia as sustained or intermittent muscle contractions causing abnormal movements or postures, and lists treatment options as drugs, botulinum toxin, and deep brain stimulation surgery.
A single case report describes a 20-year-old man with dopa-responsive dystonia and schizoaffective disorder who presented with both dystonia and psychosis after stopping medication. The report notes that behavioural changes such as depression and anxiety typically follow motor symptom onset in dopa-responsive dystonia, but that psychiatric symptoms in this disorder are understudied.

The focal dystonias: Current views and challenges for future research
AbstractThe most common forms of dystonia are those that develop in adults and affect a relatively isolated region of the body. Although these adult-onset focal dystonias are most prevalent, knowledge of their etiologies and pathogenesis has lagged behind some of the rarer generalized dystonias, in which the identification of genetic defects has facilitated both basic and clinical research. This summary provides a brief review of the clinical manifestations of the adult-onset focal dystonias, focusing attention on less well understood clinical manifestations that need further study. It also provides a simple conceptual model for the similarities and differences among the different adult-onset focal dystonias as a rationale for lumping them together as a class of disorders while at the same time splitting them into subtypes. The concluding section outlines some of the most important research questions for the future. Answers to these questions are critical for advancing our understanding of this group of disorders and for developing novel therapeutics.
AbstractBACKGROUND: Idiopathic adult-onset primary dystonia usually affects the upper body and remains focal. Underlying mechanisms are unknown, and there are only limited neuropathologic studies in the literature. Recently, ubiquitinated perinuclear inclusion bodies were found in the brainstem of patients with DYT1-related dystonia. In X-linked recessive dystonia-parkinsonism, neuronal loss in the striosome compartment of the striatum has been described. However, it was unclear whether these changes are characteristic of these particular disorders or an epiphenomenon of dystonic conditions in general. METHODS: Six cases of adult-onset dystonia and four controls were studied using immunohistochemistry to determine the presence of inclusion bodies immunoreactive for torsinA, ubiquitin, and laminA/C in the brainstem. The distribution of calcineurin expressing neurons in the striatum was also determined to ascertain whether there is loss of neurons in the striosome compartment. RESULTS: In contrast to early-onset dystonia, neuronal inclusions immunoreactive for torsinA, ubiquitin, and laminA/C were not present in the brainstem nuclei. There was no apparent loss of the striatal striosome compartment. CONCLUSION: Our findings suggest that the underlying mechanism in the adult-onset primary torsion dystonia is different from that of early-onset DYT1-related dystonia and also DYT3 X-linked recessive dystonia-parkinsonism. Alternative mechanisms may underpin the pathophysiology of adult-onset primary dystonia.
Phenotypic variability of the DYT1 mutation in German dystonia patients
AbstractPrimary dystonia is a clinically and genetically heterogeneous movement disorder characterized by sustained involuntary muscle contractions causing repetitive movements and/or abnormal postures. Recently, the gene locus (DYT1) and mutation responsible for a substantial number of cases suffering from early-onset primary dystonia was described. Here we report 2 German families and 1 sporadic patient with early-onset dystonia due to the DYT1 mutation in order to illustrate the variability of clinical manifestation within this molecularly defined entity. We demonstrate that writer's cramp or focal cervical dystonia is a clinical presentation of DYT1 as well as generalized dystonia.
Frequency of DYT1 mutation in early onset primary dystonia in Italian patients
AbstractThirty Italian patients with sporadic, early-onset, primary dystonia were screened for the DYT1 mutation. Five patients were positive (mean age at onset, 8 years); two had the typical phenotype, two a generalised dystonia also involving the cranial muscles, and one a segmental dystonia. In the other 25 patients (mean age at onset, 7.7 years), dystonia was generalised in 22 patients and remained segmental in three. Our results indicate the role of DYT1 mutation in Italian patients and confirm clinical and genetic heterogeneity of early-onset primary dystonia.

Dystonia: A review
AbstractDystonia is a hyperkinetic movement disorder characterized by sustained or intermittent muscle contractions that cause abnormal repetitive movements, abnormal postures, or both. The new consensus classifies dystonia into two axes to characterize clinical characteristics, and etiology. This system allows correct identification of isolated and combined forms of dystonia and retains the description of generalized and focal dystonia which is very useful in planning investigations and management. The characterization of dystonia for its better identification and a brief overview of its management are discussed in this article. The treatment options for dystonia include drugs, botulinum toxin and deep brain stimulation surgery.

Management of Psychosis in a Patient with Probable Dopa-Responsive Dystonia
AbstractDopa-responsive dystonia is a rare childhood neurological disorder characterized by asymmetric dystonia, predominantly of the lower limb, that responds excellently to levodopa replacement therapy. Although it is known that behavioral changes, such as depression, anxiety disorders, and sleep disturbances, typically follow onset of motor symptoms, there is limited literature on the psychiatric symptoms of this disorder. This report describes a novel case of a 20-year-old male with a history of dopa-responsive dystonia and schizoaffective disorder who presented with both dystonia and psychosis after a period of medication noncompliance. This case provides a reference for the management of psychosis in patients with dopa-responsive dystonia and highlights the need for more research on the nonmotor symptoms that accompany this neurological disorder.
