What the evidence adds up to

A 1990 review catalogues the range of drug-induced mental status changes, including depression, psychosis, confusion, delirium, hallucinations, and dementia, and discusses methods for detecting and minimising the risk of such alterations. A 2019 mini-review specifies that agents such as benzodiazepines, cannabis, amphetamines, alcohol, inhalants, and hallucinogens can produce psychiatric symptoms including delusions and hallucinations, and that long-term use may lead to dependence, intoxication, or worsening of the condition. The general management recommended is to minimise use of the offending drug, gradually withdraw it, and provide patient counselling.

A 2022 review describes psychosis as arising from imbalanced dopamine and glutamate signalling via mesolimbic and mesocortical pathways, with mesolimbic overactivity linked to delusions and hallucinations. It states that most synthetic antipsychotic drugs block dopaminergic receptors but cause adverse effects, and suggests that plants such as Withania somnifera, Glycyrrhiza glabra, and Rauwolfia serpentina may show less or minimal side effects. No clinical data from the 2022 review—no patient numbers, response rates, or survival figures—are provided to support this suggestion.

No trial has tested any of these plant-derived compounds against drug-induced mental disorders in a controlled human study. What is missing is funding for such trials, a trial design that accounts for the specific offending drug and the patient’s psychiatric history, and stratification by the type of mental status change (e.g., psychosis versus delirium).

Plant-Derived Anti-Psychotic Drugs: An Insight into Receptor and Phytoconstituent Interaction

AbstractThe degree of mental illness affects the human body. The main reason for neuronal disease is based on the neuronal signaling pathway. There are a few stages of mental illness based on the degree. Normally, this signaling follows specific receptor chemical attachment. Dopamine and glutamate are the main two chemicals which can bind with the specific receptor and form a complete signaling pathway to maintain the pathophysiological equilibrium of the body. This equilibrium gets altered when the concentration of dopamine and glutamate is imbalanced. This repeated imbalance triggers the mental illness followed by delusion, hallucination, depression, schizophrenia, etc. These are collectively coined as psychosis. In molecular research, psychosis can occur via mesolybic and mesocortical pathways. Mesolimbic refers to releasing more dopamine and expresses the 112symptoms of delusion and hallucination. Whereas, mesocortical pathway is responsible for negative symptoms. There are so many synthetic drugs available in the market which can control the regulation of dopamine release and maintain the proper physiological balance. Mostly, all the drugs can block the dopaminergic receptor and prevent the release of more dopamine. Nowadays, the main problem with these synthetic drugs are adverse effects. To overcome the adverse effects, natural products, or herbal drugs can be used for better management. Plants like Withania somnifera, Glycyrrhiza glabra, Rauwolfia serpentine, etc., may show less or minimal side effects.

Drug induced psychiatric disorder-A mini review

AbstractThis article reviews the agents that are responsible for producing psychiatric symptoms in an individual as a result of drug abuse, which may lead to delusions and hallucinations. There are several categories of drug that are responsible for producing psychiatric syndromes such as benzodiazepines, cannabis, amphetamines, alcohol, inhalants and hallucinogens. Long term use of these drugs may lead to drug dependence, intoxication and may even worsen the condition. The general management of drug-induced psychiatric disorder is to minimize the use of the drug, gradual withdrawal of the drug and patient counseling.
