What the evidence adds up to

A 1999 review of selective serotonin reuptake inhibitors (SSRIs) in intellectual disability notes that individuals with mental retardation may be particularly susceptible to dysfunction in the serotonin system, and that SSRIs appear to provide an important treatment option for target behaviours associated with anxiety and depression in this population. The review covers pharmacokinetics, pharmacodynamics, and potential effects on the developing brain, but provides no trial data, response rates, or sample sizes. A 2007 review of pharmacotherapy for pervasive developmental disorders (PDDs) states that few randomised double-blind treatment studies have been published, and very little is known about the long-term safety of these agents in this patient population. It concludes that some patients with PDDs may benefit acutely from pharmacotherapy, but many marketed psychotropic agents that hold promise have not been rigorously studied.

A 2002 textbook on child and adolescent psychopharmacology synthesises research on drug treatment for disorders including attention-deficit hyperactivity disorder, obsessive-compulsive disorder, major depression, schizophrenia, bipolar mania, aggression in pervasive developmental disorder, Tourette's syndrome, and substance abuse. It emphasises that treatments cannot be straightforwardly extrapolated from adult studies and discusses tolerability, efficacy, and appropriate use within a social and developmental context. What is still missing are adequately powered, randomised, double-blind trials with long-term follow-up, clear patient stratification by diagnosis and developmental stage, and funding to conduct such studies rather than relying on extrapolation from adult populations or small open-label reports.

Practical Child and Adolescent Psychopharmacology

AbstractThe field of child and adolescent psychopharmacology is rapidly growing, but psychopharmacological treatments for children cannot be straightforwardly extrapolated from adult studies, which presents clinicians with assessment and prescribing challenges. This important book synthesises research findings about drug treatment of a broad range of psychiatric disorders in children, including attention-deficit hyperactivity disorder, obsessive-compulsive disorder, major depression, schizophrenia, bipolar mania, aggression in pervasive developmental disorder, Tourette's syndrome and substance abuse. They examine the issues of tolerability and efficacy, and appropriate use, within a social and developmental context. For each disorder, pharmacotherapy is discussed in the wider context of neurobiology, etiology, diagnosis and treatment. This will be essential reading for all mental health professionals to inform practice and improve patient outcomes.

Selective serotonin reuptake inhibitors in intellectual disability

AbstractThere is considerable evidence linking developmental pathology with serotonergic abnormalities. Individuals with mental retardation may be particularly susceptible to dysfunction in the serotonin system. The development of more selective drugs that modify serotonergic function appears to have provided an important treatment option for a number of target behaviors, especially those associated with anxiety and depression, in persons with intellectual disability. Clinicians treating this population need to be familiar with the pharmacology of these agents, known collectively as the selective serotonin reuptake inhibitors. This article reviews the pharmacokinetics and pharmacodynamics of these compounds, as well as their potential effects on the developing brain. Recent Advances in the Pharmacological Treatment of Pervasive Developmental Disorders

AbstractIntroduction. There is growing body of evidence to suggest that pharmacotherapy may be a rational form of intervention for some patients with pervasive developmental disorders (PDDs). The purpose of this article is to describe what is known about medication treatment for these patients. Methods. Published papers regarding the pharmacotherapy of PDDs were collected and reviewed. For each study, particular attention was paid to ascertaining patient characteristics, the medication doses employed, and medication tolerability. Results. There is evidence of varying methodological rigor to suggest that several medications may have a role in the treatment of patients with PDDs. However, to date, few randomized double-blind treatment studies have been published. In addition, there is very little known about the long-term safety of many of these agents in this patient population. Conclusions. Some patients with PDDs may benefit acutely from pharmacotherapy. Many marketed psychotropic agents that putatively hold promise for these patients have not been rigorously studied. More short-term and long-term research regarding the pharmacotherapy of patients with PDDs is needed.
