What the evidence adds up to
The common cold is caused by a large number of respiratory viruses, and all of these viruses may produce the illness complex recognised as the common cold. The disease fits an "iceberg concept" ranging from asymptomatic infection to severe illness and death. Factors that influence the incidence of colds include crowding, sociability, stress, smoking, alcohol, immune status, sex, age, sleep, season, chilling, nutrition, and exercise. The mechanism of symptoms is related to the innate immune response, and the host response to the virus, particularly pro-inflammatory cytokines, appears to be an important contributor to pathogenesis and an attractive target for new interventions.
Currently available treatments for the common cold have limited efficacy against specific symptoms. A 2002 review of treatment modalities found that antibiotics, antihistamines, decongestants, anticholinergics, mast cell stabilisers, herbal medicines, zinc, and steam therapy have been studied, with studies both supporting and refuting their effectiveness. Newer antiviral agents evaluated in clinical trials may provide a starting point for development of effective therapies, but no concrete survival or response rates are reported in these abstracts. The 1941 paper draws an analogy between respiratory tract invasion and war, emphasising the importance of the nasal mucous blanket and ciliated epithelium as a first line of defence, and noting that dry hot atmosphere, metabolic disturbances, and dietary insufficiencies can break down these defences.
No vaccine is yet available, and the morbidity associated with the common cold remains a topic of discussion. The 1967 paper from the Medical Research Council Common Cold Research Unit provides no specific treatment data. The 2011 paper notes that therapies should be selected to treat the specific symptoms perceived as most bothersome by the patient, but does not report any new drug efficacy. What is still missing is a clear understanding of how to target the host inflammatory response without disrupting necessary immune functions, and the funding and trial designs needed to test antiviral or anti-inflammatory agents against the many different viruses that cause colds. Patient stratification by virus type or symptom profile has not been achieved in a practical treatment setting.

PROPHYLAXIS OF THE COMMON COLD
AbstractThere can be drawn a fairly apt analogy between war and the invasion of the tissues of the upper respiratory tract by the organisms which give rise to the common cold. In accordance with this analogy, the invading forces in infections of the upper respiratory tract are the many pathogenic bacteria associated with respiratory disease. The defenses of the nose against such invasion are twofold—the front line, which consists of the mucous blanket and ciliated epithelium of the nasal mucosa, and the second line, represented in the natural immune responses of the tissues to such bacteria. The importance of maintaining an adequate first line of defense has been stressed by Proetz,<sup>1</sup>Hilding,<sup>2</sup>Lierle<sup>3</sup>and others. Advances in treating the common cold: an update for otolaryngologists
AbstractEven though the common cold is one of the most common infections experienced by modern society, its origins, pathophysiology, and treatment remain poorly understood by the lay person and by many medical professionals. This article presents and discusses the pathophysiology of these viral infections and numerous studies that support or refute the effectiveness of several common treatment modalities, including antibiotics, antihistamines and decongestants, anticholinergics, mast cell stabilizers, herbal medicines, zinc, and steam therapy. This article also reviews several newer antiviral agents that have recently been evaluated in clinical trials and that may ultimately provide a starting point for development of effective cold therapies.

AbstractRecent studies suggest that the host response to the virus is an important contributor to the pathogenesis of the common cold. Inflammatory mediators, especially the pro-inflammatory cytokines, appear to be an important component of this response and present an attractive target for new interventions for common cold therapies. Currently available treatments for the common cold have limited efficacy against specific symptoms. These therapies should be selected to treat the specific symptoms that are perceived to be the most bothersome by the patient.
