What the evidence adds up to

Chronic lymphocytic leukaemia and small lymphocytic lymphoma are now understood to be two manifestations of the same disorder, with the cell of origin being a mature B lymphocyte that carries a rearranged immunoglobulin gene. The disease is the most prevalent lymphoid neoplasm in Europe and North America. CLL cells express only modest amounts of surface immunoglobulin and are characterised by defective apoptosis; the cause remains unknown.

A 1979 report described two patients with pre-existing chronic lymphocytic leukaemia who developed a second lymphoproliferative disorder — either histiocytic lymphoma or Hodgkin disease — which had previously been considered a terminal event. Both patients achieved an objective clinical response after their therapy was changed to a more aggressive combination of drugs. The authors concluded that any sudden change in the clinical course of a CLL patient should prompt a thorough reevaluation for a second lymphoproliferative disorder, and that if one is found, more aggressive therapy should be initiated.

A 2020 case report described a patient diagnosed with stage IV small lymphocytic lymphoma who presented to the emergency room with acute neurologic manifestations, specifically unilateral vision loss. No treatment outcome or survival data were provided for that patient.

No randomised trial has established a standard repurposed drug for CLL or SLL. What is missing is a prospective, controlled study that stratifies patients by genetic subtype and prior treatment history, and that is adequately funded to test a specific repurposed agent against current standard care.

Chronic lymphocytic leukemia in association with a second lymphoproliferative disorder: Response to chemotherapy in two cases

AbstractThe development of either histiocytic lymphoma or Hodgkin disease in association with pre-existing chronic lymphocytic leukemia has been described in the literature as a terminal event. We describe two patients in whom the diagnosis of a second malignant lymphoma was made during life and who achieved objective clinical response after a change in therapy to a more aggressive combination of drugs. We conclude that patients with chronic lymphocytic leukemia who have had a sudden change in their clinical course should have thorough reevaluation, looking specifically for the development of a second lymphoproliferative disorder. If this is discovered, more aggressive therapy should be initiated.

Small lymphocytic lymphoma and its variants

AbstractSmall lymphocytic lymphoma (SLL) and chronic lymphocytic leukemia (CLL) are in the midst of a period of huge change due to advances on several fronts. One important development is the appreciation that SLL and CLL are two manifestations of the same disorder. Throughout this review, therefore, it should be assumed that SLL is managed in a similar manner to CLL, though the studies drawn upon and recommendations made will be based mainly on publications on the diagnosis and therapy for CLL.
